The Advanced Nurse Practitioner(ANP) has been assessed as competent in practice using their expert clinical knowledge and skills. They have the freedom and authority to act and make autonomous decisions in the assessment, diagnosis and treatment of patients. The ANP provides an opportunity for patients to avoid the need to be referred to a GP, thereby enabling them to receive timely care and negate unnecessary delay in receiving treatment. ANPs in primary care can develop close, long-term relationships with their patients and work in partnership with them to achieve optimum health. They are autonomous in making decisions based on assessment, diagnosis, and interpretation of test results. Primary Key Responsibilities The following are the core responsibilities of the Advanced Nurse Practitioner. As a Long Term Conditions Nurse you will be responsible for ensuring a comprehensive handover of services when patients are transferring across wings or custodial care settings. You will identify immediate health needs including, any external services or prescribing requirements that prisoners may have received e.g. prior to remand or repatriation onto a wing A key part of this role will include the facilitation of arrangements between agencies involved in delivering a complex care package to ensure effective collaboration and stability for the patient in delivery of that care. This requires close joint working with other in-house healthcare services and departments within the prison such as Pharmacy, Education, Offender Management, Substance Misuse and Physical Education, etc. You will also; Provide proactive intervention to urgently address improvement recommendations from CQC and HMPP inspections Deliver continuity of care across health and social care agencies from courts, to custody and community whilst improving the communication between the above.
