We are looking to recruit a knowledgeable and dedicated Research Nurse/Practitioner to join the Research and Innovation department of Frimley Health NHS Foundation Trust.

The post holder will work with a vibrant team of research nurses and practitioners delivering a broad portfolio of research studies. The post holder will be expected to contribute to the assessment and management of the care pathways for patients and carers participating in clinical trials. This will include the recruitment, education and monitoring of trial patients and the collection and documentation of accurate data.

Previous research experience is desirable but not essential. Full training and supervision is provided. Experience in any of the specialty areas such as cancer, urology, critical care or midwifery would be of advantage.

The post holder will: 1. Be able to combine high level remote monitoring with telephone assessment for patients with short and long-term conditions. Intervene when markers for deterioration are evident. Subsequently, initiating medical and/or nursing/therapy treatments, by alerting and referring to primary care multidisciplinary teams and professionals. PATIENTS CARE The post holder will: 1. Use advanced clinical skills to remotely monitor and assess the physical and psychosocial needs of a defined client group. 2. Use advanced generalist clinical skills to identify subtle changing health care needs of patients. Subsequently, being able to competently discuss treatment options with other generalists and specialists. 3. Demonstrate responsibility and accountability for the Digital Health clinical caseload and escalate care needs across the whole patient pathway when this is required. Analyse complex patient situations establishing a therapeutic relationship to assist the individual patient to adjust to their illness and care with the Digital health service. 7. Use assessment tools/skills that will ensure an appropriate level of nursing or therapeutic intervention so that patients who present with complex needs are referred/escalated via the appropriate care pathway. 8. The Berkshire Early Intervention in Psychosis Service (EIP) is looking for a passionate and enthusiastic band 6 mental health practitioner to join their innovative and dynamic team. The EIP team is a trust wide service, with teams based at each locality. We are a specialist service working with individuals experiencing a first episode of psychosis or who are presenting with an at-risk mental state, between the ages of 18 to 65. We operate a Monday to Friday 9am to 5pm service, although some flexibility is required. This advert is for a role covering Wokingham locality, based at London House, Bracknell. You will have a clear passion for supporting people and their family in their recovery. You will work with our integrated team of psychiatrists, psychologists, psychological therapists, and STR workers to empower and support our clients to understand their experiences and return to college, university or employment. We work with our clients for up to 3 years and the majority do get transferred back to their GP, therefore it is a rewarding team to work with.
• To act as care coordinator, in accordance with the Care Programme Approach (CPA) policy under the supervision of the Clinical Lead. The role is focused on working collaboratively with the client and their family/friends on their recovery, developing a relapse management plan.
• To undertake comprehensive assessment of health and social needs together with the client and their family to implement a plan of care andprovision of NICE recommended interventions.

The integrated care practitioner role is well established within Bracknell. If you are energetic, a team player with a good skillset for autonomous working and decision making; you will be the right person to come and work as part of the Integrated Care Team.

The role is truly situated in between the services to prevent people falling between the gaps; you will be able to guide the person on correct pathway so there are “no wrong doors”. You are working closely with community nursing colleagues as well as local authority colleagues from Bracknell Forest, discussing new referrals and completing home visits.

This is a hybrid role, whilst being able to work from home, you’ll also have opportunity to work at Bracknell CMHT OA, Church Hill House and Bracknell Forest Council, Time Square.

You will be a well-respected member of Bracknell CMHT for Older Adults and the Integrated Care Team, this is a matrix management model - your operational supervision will be provided by the service manager of the Integrated Service, your clinical supervision will be provided from Bracknell CMHTOA.

Your caseload will come from the work you undertake within the Integrated Care Team, you will work closely both Adult Social Care and CMHTOA in Bracknell, you’ll attend referrals meeting to provide steer as part of the integrated work.


• To represent BHFT mental health services at locality LAP/Cluster/Care Home meetings. To screen referrals, identify any further information which needs more exploration to be able to identify a correct pathway.
• To provide clinical leadership, and offer mental health expertise & guidance to professional colleagues and assist in formulation to support integrated decision making and care provision
• To meet the multiple and often complex needs of frail patients with long term conditions who are discussed, through on-going joint assessment of health and wellbeing, including risk assessment, developing, implementing and evaluating intervention plans.
• To support the multidisciplinary team to identify which patients discussed could benefit from mental health intervention.
• To work as an autonomous practitioner, taking responsibility for mental health assessments establishing a formulation and implementing the clinical management required.
• For those patients not requiring secondary mental health input, to support navigation of the health and social care system by recommending the most appropriate level of psychological support from the range of provisions available.
• To hold a small caseload requiring more complex mental health support.

We are looking for strong team players who have a real passion for working with children and families. You will need to have excellent communication and interpersonal skills with the ability to work with other teams and agencies.

The role is 9 - 5 Monday to Friday in which we can offer some flexibility. This is a client facing role but with some room for home working based on service need.

Main duties of the job

Working collaboratively with other health care professionals and agencies ensuring a seamless delivery of service for children and families within the community.
Assessing child and family needs, using the appropriate assessment and risk tools. Ensuring that at each contact with a child an assessment of their health and development need is conducted
To support breastfeeding and parent infant relationships applying the Unicef UK Baby Friendly Initiative standards
Ensure that the advice, support and services provided take account of client or family's race, culture, religion and language
Full knowledge and understanding of the Local Safeguarding Board Procedures together with Trust guidelines. Exercises vigilance regarding vulnerable families and to respond as necessary
Working closely with local authority colleagues

Requirements

Be a registered Specialist Community Public Health Nurse (Health Visiting) active on the NMC register
Previous experience managing cases where a child has a protection plan
Have a high standard of clinical work
Have a valid UK driving license with access to vehicle

We also offer a £250 bonus for starting this contract this will be paid out to you after 150 hours completed (4 Weeks)
