DaVita is looking for a Full-Time Nurse Practitioner (NP) to join our Nightingale Clinical Care Team in the Sacramento, CA market.

In this role, you will provide comprehensive healthcare across the many co-morbid chronic conditions common to chronic kidney disease (CKD) and end-stage kidney disease (ESKD) patients within a designated market or geographical region. CKD NP’s place of service is predominantly via telehealth and ESKD NP’s place of service is primarily in the dialysis clinic but may vary (e.g., clinic, telehealth, and in-person, home). The NP exercises discretion and independent judgement in the performance of professional duties and functions autonomously, yet in contribution with all members of a patient’s healthcare team to provide, coordinate, and facilitate quality, cost-effective care while minimizing fragmentation of the healthcare delivery system for patients. The NP assumes a clinical leadership role and coordinates activities with the healthcare team and healthcare systems to meet the health needs of their patients. The NP will utilize an electronic health record (EHR) such as Cerner or Epic for patient documentation and scheduling, which will be reviewed by the NP Manager. You may be asked to provide services to patients outside of their assigned pod, either in-person or telephonically as needed.

Essential Duties and Responsibilities

• Performs enhanced annual wellness visits or continuous health evaluations (CHEs) to identify and evaluate patients’ co-morbid conditions and identify and assist with patients’ needs including but not limited to medical, social, emotional, and financial concerns through medical record review, consultation with patient, family, and healthcare team members.
• Manages the care of CKD/ESRD patients, using the enhanced annual wellness visit or CHE as a foundational history and physical, focusing on medical cost/utilization and preventing/reducing hospitalizations, and re-admissions through proactive post-discharge planning and follow-up with patients most at risk.
• Leads the clinical team and ensures care management and care coordination activities complement and support medical treatment planes while integrating patients’ psychosocial and physiological needs.
• Documents findings in patient medical record using an EHR system, ensuring that appropriate diagnoses are evaluated and accurately documented. Uses EHR calendar function for all work-related activities.
• Communicates and collaborates with DVPs, Directors, Nephrologists, other providers, Nightingale NPs, in-center hemodialysis teams, DaVita IKC employees, and medical directors regarding the healthcare needs of the patients.
• Travel: Local travel up to 100% as defined by geographical region

Preferred Experience

• 0-2 years of NP Experience; experience in population health, Primary/family care, and/or nephrology a plus

POSITION SUMMARY

PURPOSE AND SCOPE:

A Certified Registered Nurse Anesthetist (CRNA) in an advanced practice registered nurse (APRN) who practices both autonomously and in collaboration with a variety of health providers on the interprofessional team to deliver high-quality, holistic, evidence-based anesthesia and pain care services. The CRNA cares for patients at all acuity levels across the lifespan in a variety of settings for procedures including, but not limited to, surgical, obstetrical, diagnostic, therapeutic, and pain management.

PRINCIPAL DUTIES AND RESPONSIBILITIES:

• Perform and document the pre anesthesia assessment and evaluation of the patient.
• Review, request, and order (as permitted by state law), laboratory/diagnostic studies.
• Obtain informed consent for anesthesia and related services.
• Select, obtain, order, and administer preanesthetic medication or treatment.
• Develop and implement an anesthesia care plan.
• Select, obtain, prepare, and use equipment, invasive and non-invasive monitors, supplies, and drugs for the administration of sedation, anesthesia, and pain management services; perform safety checks as needed.
• Select, order, obtain, and administer analgesic and anesthetic agents, adjuvant drugs, accessory drugs, fluids, necessary to provide anesthesia services.
• Document a time- based record of events during peri-anesthetic care
• Perform all aspects of airway management, including fiberoptic intubation.
• Facilitate emergence and recovery from anesthesia by evaluating patient response, and selecting, ordering, and administering medications, fluids, and ventilator support to maintain patient stability during transfer.
• Initiate and administer post-anesthesia pharmacological or fluid support of the cardiovascular system.
• Initiate and administer respiratory support to ensure adequate ventilation and oxygenation in the post anesthesia period.
• Evaluate the patient’s response to anesthesia and surgery; take appropriate corrective action post anesthesia.
• Provide post anesthesia follow-up evaluation care
• Discharge the patient from the post anesthesia care unit
• Implement acute and chronic pain management modalities
• Manage emergency situations by assessing, stabilizing, and determining disposition of patients, including providing airway management, administration of emergency
• Place and manage regional anesthetic techniques, as delineated by his or her clinical privileges or granted special privileges.
  • Insert and manage peripheral, arterial, pulmonary catheter, or central venous lines and catheters.
  • Provide patient and caregiver education.
  • Consult with respiratory care providers and Implement respiratory and ventilator care when appropriate. 
• Supports patient safety when performing his or her job functions and through participation in facility, department, or unit patient safety initiatives.
  • Takes action to correct observed risks to patient safety.
  • Reports adverse events and near misses to appropriate management authority.
  • Identifies possible risks in processes, procedures, devices and communicates the same to those in charge.
  • Using an interdisciplinary approach, including consultation or referral to another clinician, as appropriate.

• Perform advanced and interventional pain management using drugs, regional anesthetic techniques, or other accepted pain relief modalities with or without fluoroscopic guidance.
• Utilize ultrasound, fluoroscopy, as appropriate. 
• Participates in center quality meetings, medical executive meetings and other management meetings upon request from the Medical Director.
• Review and comply with the Code of Business Conduct and all applicable company policies and procedures, local state and federal laws and regulations

EDUCATION AND REQUIRED CREDENTIALS:

• 5 years CRNA experience preferred

• Ability to operate, use, and interpret data from a wide variety of equipment to perform anesthesia care as well as necessary perioperative or operating room care to support the safety and wellbeing of the patient while they are under the direct care of the CRNA.

Reporting to the Director of Care , the Licensed Practical Nurse (LPN) is responsible for providing planned, patient-oriented, goal-directed nursing care in accordance with the provincial standards of care.

Qualifications

POSITION SUMMARY
This position is responsible for assisting clinicians and nursing staff in providing medical care, as well as, implementing and evaluating direct patient care. This position utilizes specialized knowledge, judgment and skills to provide an excellent patient experience.

CORE FUNCTIONS

Performs basic maintenance of equipment and coordinates needed maintenance for prompt repairs.

6. Orders supplies as directed, stocks patient care areas. Medication ordering and tasks related to all medications (e.g. expired medications/equipment and recalls).

7. Develops and maintains effective communication mechanism with all external and internal customers. Exemplifies service orientation and the team concept in all contacts.

8. This position will complete all necessary on-boarding/orientation activities, including simulation training, as needed or directed.

9. This position works under the direct supervision of the providers. This position interacts with all levels of staff in a variety of departments, physicians, patients, families and external contacts, such as employees of other health care institutions, community providers and agencies, concerning the health care of the patient. Also interacts with physician’s offices in order to report, ask for or clarify information. Prioritizes data from multiple sources to provide support for the response of the patient and family to changes in health status.

MINIMUM QUALIFICATIONS

High school diploma/GED or equivalent working knowledge. Completion of an approved medical assistant training program as defined by state regulations.

Active Medical Assistant Certification is required. Certification or additional training must meet the requirements for Meaningful use. BLS certification required. • Inspects operation of all facility equipment and reports malfunctions, and maintenance needs to ensure the safe and effective operation of all vascular access equipment.
• Inspects operation of all facility operating systems, including temperature checks in appropriate areas and physical structure of the facility and reports any issues to the Facility Manager.
• Ensures a clean, safe and sanitary environment in the vascular access facility at all times.
• Ensures all blood spills are immediately addressed according to the organization/FMCNA Bloodborne Pathogen Control Policies.

ADMINISTRATIVE:

• Assists with efficient utilization of medication, laboratory, inventory, supplies and equipment to achieve supply cost goals following all guidelines established in the organization formularies.
• May assist in the development and revision of any applicable policies and procedures related to the facility to ensure continuous process improvement.
• Assists with Continuous Quality Improvement (CQI) activities including monthly reports and attending meetings as required.
• Assists with maintaining medication supply, including inventory control, checking for expired medications, and ensuring appropriate recording of controlled substances as required by law.

Requires national certification from the American Registry of Radiologic Technologists and licensure by the state regulatory agency if applicable. BLS certification required. Requires successful completion of the Banner Health MA competency assessment. We are looking for a self-motivated, organized individual who is passionate about patient care and excited for the opportunity to independently manage their cases with confidence and accountability. As a Peritoneal Dialysis Registered Nurse (PD RN), you will be providing education on peritoneal dialysis to your patients and their care partners in a clinical setting, which then allows them to dialyze from home.

You have responsibility over the complete ongoing care of each patient to ensure their treatment stays on track. 

Dialysis Experience is NOT required.  Nephrology nursing provides a fresh lens and set of skills to those newly entering the field.
