· The post holder will contribute to the effective operation of a multi-disciplinary Home Treatment Service for those in mental health crisis in the community, as an alternative to hospital admission.



· The post holder will provide rapid OT assessment and help plan and deliver effective evidence based home treatment in collaboration with service users, carers and professionals from other services and agencies.





· There will also be a role for the post holder at the recovery house in Enfield for those clients needing extra support. The post holder will have responsibility for the group programme of the unit and will be expected to provide clear assessment and treatment plans for clients using evidence based interventions including group work and 1:1s at the recovery house.



· The OT will also be an integral part of the wider Inpatient Therapy Team at Chase Farm Hospital and will receive supervision and support from the Lead OT

The post-holder will primarily offer liaison to the Acute Wards in Enfield relating directly to the service users with a diagnosis of Personality Disorder who are being considered for admission or already an inpatient . This will include attending ward rounds and team meetings to offer trauma-informed consultation and clinical advice to Acute teams as well as delivering training and supervision.

The role aims to use experience and clinical knowledge to increase the capacity of the teams delivering care to meet the needs of this service user group as well as enhance the experience of the service users in the least restrictive environment possible. Therefore, understanding risk and a confidence in discharge planning is an essential skill for the post holder. The post holder will undertake urgent assessments on behalf of the Personality Disorder Pathway, supervising team members from the ward and the Community Teams to take up the outcomes and recommendations of these.

The post holder will lead on ensuring that service delivery focuses on the quality agenda and that dignity, privacy and respect is afforded at all times to service users. They will also support the development, implementation and embedding of trauma informed practice on the Acute wards.
• To provide skilled trauma-informed assessments of patients’ emotional and psychological needs based upon the understanding, interpretation and integration of complex data given by the patient and obtained from other sources
• To evaluate and make decisions about trauma-informed treatment plans in conjunction with the MDT

We need 2 x full time (35hrs weekly) Senior Enablement Workers to cover 2 vacancies This role plays a vital role in ensuring each Adult we support has a support plan following assessment of needs completed at the point of hospital discharge, in to their own homes The role supports our requirements under our CQC regulated activity of personal Care and delivering a safe service. Applicants must have experience of putting together a care plan along with risk assessments and setting up medication Mar sheets within each client's home, so that our carers know how to support the clients safely. candidates MUST have access to a car and willing to drive around Enfield to do home based assessments, working on a rolling rota to include working one weekend in 4. THIS IS NOT A SOCIAL WORKER ROLE. we pay mileage to each worker in addition to the hourly salary. The worker will have on average 3 home visits daily to put
together a care/support plan.
