A Diabetes Specialist Nurse requires expertise in the management of diabetes in both hospital and community settings. The role is educational and advisory with the aim of providing specialist diabetes care to the person with diabetes, their carer and training for healthcare professionals.

We invite applications for an enthusiastic and compassionate new team of five mental health and wellbeing practitioners to work within our community mental health services across Coventry and Warwickshire.

This is an exciting, brand-new role created by Health Education England and NHS England and Improvement to support the transformation of adult community mental health services. Your training will be fully funded by the NHS, and you will be eligible for a full-time qualified position if you pass the course. These posts are suitable for both graduates (e.g., psychology) and non-graduates, who have the right mental health experience and personal qualities. This role will contribute to ensuring adults with severe mental health problems have greater access to psychologically informed interventions.

What Grade Will I Be?

As a trainee mental health and wellbeing practitioner, you’ll be paid on the Agenda for Change (AFC) pay system commencing at Band 4. After qualifying, you’ll be paid at Band 5.

You will be involved in the delivery of psychological interventions within a community mental health setting.

The role involves seeing adult patients, in person or via video link, who have been referred by GPs and other local services and providing psychological support for them in collaboration with the multidisciplinary team. This will include providing the following psychological interventions:
• behavioural activation and graded exposure using the ‘GOALS’ programme
• problem-solving
• improving sleep
• recognising and managing emotions
• confidence building
• support with managing medicines management

The successful applicant will work alongside a dynamic, supportive multi-disciplinary team including a large number of other nurses, psychiatrists, support workers, occupational therapists, social workers and psychologists to deliver recovery focussed interventions in the community. This includes key working and generic roles in the team such as new patient assessments and duty, as well as specific nursing tasks. The training: Youll undertake a one-year training programme,spendonedaya weekstudyingat university, some time for private study and the remainder of the time(three to four days)working inor undertaking specific training tasks ina community mental healthservice. As a trainee mental health and wellbeing practitioner, youll be paid on theAgenda for Change(AFC)pay system at band 4. After qualifying, youll be paid at band 5. We are developing a new team to work with patients who have mental health needs with associated forensic risks.

The team supports individuals with a history of being in prison or secure services and doing preventative work for those at risk of prison or secure services due to their mental health.

Our new team is taking shape, and we want this to be a truly integrated and multi-disciplinary service. We are currently proposing a model of OT, Nursing, Psychiatry, Psychology, Social Worker, Voluntary sector, support worker, peer worker as well as dual diagnosis specialists around substance misuse and a social care funded autism post that will support the team.

The role has the expectation of managing and / or supervising junior staff and providing leadership for the team, your leadership skills will also be assessed in interview.

You will work closely with clinical leads, managers and colleagues to develop the service and manage a caseload of people as well as provide advice and support for other services. You will also joint work with individuals supported by other services to provide support around risk management. You will help develop treatment pathways, processes and structures as well as building effective multiagency links to ensure that we meet the diverse needs of the people we see.

You will provide assessment and interventions to individuals as well as complete outcome measures to show progress made and identify areas of needs. You will ensure that the needs of the people you see are met and that will involve drawing in other staff in support of this.

Community Mental Health Nurse - Band 5

We have an exciting opportunity for a band 5 nurse to gain experience in the area of Community Mental Health in the Coventry Community Mental Health Team. The team works with service users with psychotic and non-psychotic illnesses across the city of Coventry and are based at Swanswell Point.

The successful applicant will work alongside a multi-disciplinary team including other nurses to deliver recovery focused interventions in the community. This includes key working and generic roles in the team such as office duty as well as specific nursing tasks such as supporting our range of clinics, therapy and group interventions. We encourage staff to bring their own ideas to meet patient needs, and have regular service development sessions to encourage ongoing development. You will receive extensive support and supervision from the Management and Leadership team.
