The Greater Manchester Homeless Hub has an exciting opportunity for a dynamic and enthusiastic individual to apply for this newly developed role, to improve the service delivery within the Manchester Mental Health Homeless Team.

The successful candidate will play a central role in supporting the clinical and operational team to ensure high quality trauma informed care is delivered to our service users.

The post holder will lead assessment, formulation and prescribing of care / treatment planning, including discharge planning and transfers of care within agreed protocols and procedures and be an integral part of the MDT supporting clinical decision making and supporting the service pathways.

The post holder will provide advanced and expert level of practice (including non-medical prescribing) within the Manchester Mental Health Homeless Team leading to improved patient outcomes.

The post holder will support the clinical and operational team to deliver on service improvements in the quality of care and safety through service development, evaluation, and audit.

They will be an integral part of the MDT supporting clinical decision making and supporting the service pathways, leading on the physical health offer across the Homeless Hub . The post holder will also support the development of the CTI Model (Critical Time Intervention) within the team by providing a seamless pathway into & out of the CMHT, once interventions / goals have been achieved, to promote recovery and self-management, as clinically appropriate.

The Advanced Practitioner will hold a case load and will lead assessment, formulation and prescribing of care planning including discharge planning within agreed protocols and procedures.

The job holder may additionally lead on practice and service developments. The job holder is required to demonstrate clinical and practice expertise via interventions, supervision and modelling good practice.

To support improving workforce knowledge and practice through the contribution to education and training for the team.
