This position functions in the clinical associate role in the primary care clinic, which utilizes a Patient Aligned Care Team (PACT) model. The PACT ICT is responsible for providing direct and/or indirect patient care to an assigned group of patients. She/he will have the knowledge, skill and experience to carry out assignments involving point of care testing, providing treatments and vaccinations, performing diagnostic studies, and patient/family education. Major duties include:

• The incumbent assists outpatient licensed staff with the delivery of care to patients with complex, specialized, and potentially life-threatening situations.

The Lead Nurse for the Thames Valley and Wessex Paediatric Critical Care ODN will provide nursing leadership across the Network, acting as a source of expertise, advice and knowledge for providers, commissioning and ICBs. You will work alongside the national PCC ODN lead nurses to deliver on nationally agreed programmes of work and to support regional surge planning for PCC capacity.

The role of pharmacy within General Practice is part of an exciting programme of transformation which is underpinned by the NHS Long Term Plan. This is an innovative role to work within the Lakeside Healthcare Pharmacy team to ensure safe and effective use of medicines for patients within Lakeside Healthcare. As a starting point the role will initially focus on medicines reconciliation from clinic and discharge letters, and will develop further as the role of the Pharmacy team expands within the organisation. It is vital that effective systems and procedures are in place to ensure that this process is effective, safe and efficient. The right candidate will be involved in day to day management of this with regular and thorough reviews of all medications. The process also ensures that patients receive the appropriate monitoring necessary for the medication they are prescribed. Job Responsibilities/Principal Duties to include: Medicines reconciliation from clinic and discharge letters Take a central role in the clinical aspects of shared care protocols Level 1 medication reviews to include: Identifying patients on repeat medication who have not had a medication review Check medical notes to assess if any monitoring is required and arrange as appropriate Update medication as appropriate following defined protocols Remove any old or unnecessary medication from repeat Update quantities to provide appropriate supply each month Review dosage instructions to ensure concise and clear instructions Ensure prescribing is in-line with the Lakeside Drug Formulary whilst taking consideration of CCG prescribing guidance Support Clinical Pharmacists with general prescription requests and medication queries to allow them to review the more clinically complex requests Liaise with the pharmacist if there are any concerns regarding patient medication regimens Promote and support the implementation of electronic repeat dispensing Conduct searches for audits and high-risk drug monitoring using clinical systems. Participate in practice meetings, and other meetings to improve engagement of the role of pharmacy technician within the Lakeside Pharmacy model and to promote issues relevant to prescribing. Undertake training and development, to ensure the required knowledge and skills for practice-based work. Support initiatives for antimicrobial stewardship to reduce inappropriate antibiotic prescribing. Assist in the delivery of medicines optimisation incentive schemes and audits relating to MHRA alerts, drug monitoring and other patient safety initiatives. Develop relationships with other pharmacy technicians, pharmacists and members of the multi-disciplinary team to support integration of the pharmacy team across health and social care including primary care, community pharmacy, secondary care, and mental health. Training and personal development: You will participate in any training and development programmes implemented by Lakeside Healthcare. This could include: Participation in individual performance reviews, including maintaining a record of your own personal and professional development. Working with the Senior Pharmacist and the Management team to assess own learning needs and undertaken any training as appropriate. Our stroke medical team at North West Anglia NHS Foundation Trust is offering an exciting opportunity to a research driven Specialty Doctor to join the existing base of consultants and doctors on a permanent role. You will require to work cross site at Peterborough City Hospital and Hinchingbrooke Hospital. This is a fixed term post until 30 April 2025.

Working in collaboration with our specialist team, you will provide comprehensive inpatient and outpatient stroke services to our local communities such as Hyper-acute care including thrombolysis, acute care and rehabilitation.

You will enjoy a high level of autonomy and support from your team, and you will share your specialist expertise while provide a stroke consultation and advisory service to medical colleagues in other specialties in the Trust, potentially in other locations.

The job plan is anticipated to contain 10 programmed activities per week. We are prepared to negotiate any changes on the job plan for the benefit of clinical service delivery and the department as a whole.

You will join the on-call 1:4 twilight shifts between 2pm to 10pm covering thrombolysis and referrals, including a 1:4 weekend rota.

Supported by a team of 4 stroke consultants and a multidisciplinary team, you will be responsible for stroke thrombolysis, TIA clinics, inpatient referrals, ward rounds and MDT under consultant supervision as per an internal rota.

As part of your professional development, you will participate in current and future research programmes as well as in service developments in line with national targets and locally commissioned improvements.

As a Pre-registration Trainee Pharmacy Technician apprentice, you will undertake an approved Level 3 or level 4 qualification (dependant on knowledge provider). As the programme is delivered as an apprenticeship, you will be expected to spend time in the workplace and 20% off the job learning.
