The Recovery Focussed Team is an all age adult ( 18+) Secondary Mental Health Community Team providing care to individuals with a variety of mental health problems including psychosis, complex emotional needs and organic conditions. We are always looking at developing the service and pathways we provide; so this is an exciting time to join the team in this period of ongoing development.

We offer strong clinical support, supervision and multidisciplinary team working. We pride ourselves on being a supportive team that welcomes innovation. The last two agency nurses who worked with the Team, liked us so much they secured permanent posts and stayed!

The Trust has a well-established appraisal and supervision structure and actively encourages continual professional development.

We welcome students to the team, and always get excellent feedback on their experiences whilst on placement with us.

The working hours are Monday to Friday 9-5 (some flexibility required) and excluding bank holidays.Staff from the complex emotional needs pathway will be available on Teams on the Monday and Friday at 12:30 pm for informal discussions whilst the advert remains open.

Applicants must demonstrate a flexible approach, evolving around the needs of service users, their families and significant others. The role involves working as a team member, with the responsibility of a caseload within the complex emotion needs service with the CMHT.

You will beresponsible for the assessment, planning implementation and evaluation of the evidence-based care.

The service ethos is focused on recovery and social inclusion and therefore offers successful applicants an exciting and demanding opportunity to improve people’s lives.

We are looking for an enthusiastic person with excellent team working skills, who is keen to develop their knowledge and skills in this area. You should be a competent and motivated practitioner with appropriate demonstrable experience.

About You:   Become a Clinical Assessor (Nurse Assessor) with Medacs Healthcare and use your clinical knowledge and expertise in a new and rewarding hybrid role based out of Scunthorpe.

As an experienced registered Nurse Assessor, you will use your clinical skills to assess, discuss, and create detailed reports on how their conditions impact daily living by conducting face to face and telephone assessments.

Essential Qualifications:

An exciting and challenging opportunity has arisen for a dedicated, enthusiastic individual to join our administrative team to provide secretarial\clerical support to the Psychiatry Medical teams within the Learning Disabilities & Forensic Directorate.

The applicant will be the central point of contact for the Psychiatry team in North Lincs and will work alongside the current Medical Secretary in Doncaster to provide 2 days support, the successful candidate will also provide annual leave / sickness cover to the Doncaster medical support team where applicable. The environment is very fast paced and the ability to work to tight deadlines is essential. The applicant must also work well as part of a team to provide comprehensive administrative support to ensure the service runs efficiently and productively.

The post holder will be required to liaise with our service users and family members, managers and staff at all levels, and other multi-disciplinary agencies, therefore candidates must have good verbal and written communication skills. Audio Typing is a big part of this role, so it is essential that you have experience in this field.
• Clinical Support
To prepare clinic lists, room space and all necessary documentation prior to clinic sessions
• Diary management for the consultant and members of the medical team including all aspects of meeting bookings and MDT meetings and ensure consultants and members of the medical team are fully prepared for meetings
• Set up and maintain SystmOne rotas
• Effective organisation and scheduling of clinics and appointments; cancelling, re-arranging and sending letters as appropriate
• Book home visits including liaison and contact with patients/service users and other professionals
• Arranging cross cover for consultants and members of the medical team for clinical sessions as required
• Record and maintain consultant and members of the medical team activities as appropriate
• Typing of reports specific to service area i.e. MH tribunal Reports, Discharge Summaries, Section 136 Assessments, Inter ward Transfer Reports, Daily Progress notes and Inter transfer Discharges within agreed timescales.
• Typing of letters to GPs, other general correspondence and clinic sessions sometimes of a distressing nature
• Audio typing of consultant/medical correspondence, sometimes of a distressing nature
• Organise/obtain/ return medical case notes from other agencies and ensure that the appropriate monitoring system is maintained

The Primary Care Network Mental Health Team has an exciting opportunity for an experienced mental health nurse to join our team.

The pathways within Primary Care Network Mental Health are: Routine Pathway, Dementia Pathway, Medication Management Pathway, Mental Health and Wellbeing Pathway, and Severe Mental Illness Physical Health Check pathway. These vacancy is within the routine pathway, but we are an integrated team, so awareness of all pathways is required.

The routine pathway accept all routine referrals into mental health services. The role involves triage, assessment, signposting and offering brief interventions in line with the primary care model. The post holder will be expected to assess risk and ensure any urgent referrals are actioned by the relevant team.

The ARRS mental health role in primary care is an exciting opportunity to build new models of working, with community-based roles situated at the heart of General Practice while been employed by RDaSH. A GP integrated mental health service will create better joint working across primary and secondary care mental health systems.

Provide a lead role in supporting Primary Care Networks in triaging mental health concerns and providing specialist advice.

Support GPs within the network to make appropriate referrals and support bridging a gap between primary care and secondary services while working in partnership across the system.

To act as first point of contact for GPs to gain advice and support regarding people’s mental health.

Where appropriate liaise with the GP team and the PCN regarding complex situations.

To be available to participate in primary care practice level meetings, including any meetings involving serious concern, i.e., safeguarding and risk management.

Be effective in multi-professional team working and service transformation.

To provide a comprehensive Primary Mental Health Nursing Service across the whole range of patients, service will be provided mainly via clinics and one-to-one interactions with service users which will include the provision of bio-psycho-social assessment and treatment delivery, including the role of IndependentNon-medical Prescribing.

To work at this level, the nurse must be able to practice autonomously and hold the Independent Non-medical Prescribing accreditation and be approved by the Trust to work independently or be prepared to work towards it immediately when commenced in post.

Under the supervision of registered nurses, ensure patients are helped to move position to prevent discomfort and / or pressure ulcers and promote effective respiratory function. 8 Assist patients with safe mobilisation according to their plan of care and where appropriate seek help from other members of the hospice team. 9 Utilise appropriate moving and handling aids to reduce the risk of injury to the patient and themselves 10 Assist in supporting therapeutic recreational activities for patients as appropriate 11 Contribute to the assessment, implementation and evaluation of care. 12 Communicate effectively with all staff members. 13 Help to ensure a high standard of general cleanliness of the unit. 14 Observe patients and report any concerns/changes promptly in condition, to the nurse responsible for the patients care. 15 Take responsibility for the removal and safe disposal of clinical waste. 16 Participate in audits following support or training e.g. hand washing, environmental observations. 17 Ensure privacy, dignity, and confidentiality for patients and families is maintained at all times. 18 Support trained nurses in their roles as instructed. 19 Participate in end of life care which will include Last Offices 20 Participate in social activities to offer further support to patients and families. 21 Assist registered staff in the taking and recording of vital signs as per the patients plan of care 22 Be aware of and contribute to the protection of individuals from abuse and report any suspicions of abuse to the nurse in charge 23 Have a duty and a responsibility to protect vulnerable adults, children and young people in accordance with National Guidelines, local policies and training. 24 Be aware of the framework for the Mental Capacity Act (2005 25 Contribute to the emotional support of identified patients who may be anxious or upset. 26 Recognise and promote the patients rights, cultural beliefs and values Communication 1 Communicate with patients and relatives in a courteous, caring, sensitive and appropriate manner, and refer to the nurse in charge as required 2 Report any incidents, accidents, concerns, near misses or complaints to the nurse in charge or using the VANTAGE System 3 Maintain good relationships with all colleagues in the hospice community and hospitals. 4 Attend relevant unit meetings and contribute to the Clinical Supervision sessions provided by the hospice. 5 Be aware of the reputation of the hospice and ensure that it is upheld. 6 Demonstrate positive behaviours and effective communications, particularly as practices and changes occur. 7 Do not accept gifts from patients and families without discussion with the Senior Nurse. Education 1 Understand that personal development is the responsibility of the individual. 2 Participate where appropriate, in helping to develop the skills and knowledge of yourself and others. 3 Support and help registered nurses in the development and orientation of other HCAs, learners and students within the team. Participate in educational programmes when required and continuous training. 4 Undertake and satisfactorily complete mandatory training and Participate in own annual performance appraisal. 5 To take responsibility for being up to date with current policies and procedures and know where and how to access these of the Lindsey Lodge L Drive. 6 Co-operate fully in the introduction of any new technology and methods as appropriate. 7 Undertake and satisfactorily complete mandatory training and annual performance appraisal (PADR). 8 Successfully complete all training programmes and relevant study days as required Other Responsibilities 1. Be responsible for maintaining and promoting the tidiness and cleanliness of all patient areas, including equipment rooms and storerooms, and take appropriate care of furniture and equipment. 2. Be aware of the cost of equipment and clinical consumables and take responsibility for safe and appropriate use 3. Ensure beds and equipment are properly cleaned between patient use in align with infection control standards 4. Adhere to infection control policy and procedures demonstrating an understanding of universal precautions and good hand hygiene 5. Undertake clinical skills in which you have received training and have been deemed to be competent in by appropriately trained and experienced registered practitioner as per Trust guidance. Be of smart appearance and wear the correct uniform in accordance with the Hospice Uniform and Dress Code policies
