An exciting opportunity has arisen for a motivated and enthusiastic pharmacist with excellent clinical and interpersonal skills to lead a high-quality clinical pharmacy service to Adult Diabetes Services at UCLH.

Adult Diabetes Services are part of the Medical Specialties Division, which incorporates a wide range of different clinical specialties, with both an inpatient and outpatient focus.


The Adult Diabetes Service consists of a multidisciplinary team of consultants, specialist registrars, associate specialists, clinical nurse specialists, dieticians and podiatrists working together closely to support and improve the health of people with type 1 and type 2 diabetes across inpatient and outpatient care settings.

The post holder will work as part of the Medicine and Emergency Services pharmacy team to deliver a comprehensive clinical pharmacy service to Adult Diabetes Services, and where relevant the Medical Specialties Division, and collaborate with primary care and other organisations across the North Central London Integrated Care System, to improve patient care in hospital and community settings.

You will lead the day-to-day clinical pharmacy service to the UCLH Adult Diabetes Services, working closely with the Adult Diabetes team and wider MDT to improve the care of patients with type 1 and type 2 diabetes.

You will attend relevant MDT clinical huddles and ward rounds to promote safe practice by reducing medicines related harm during transitions of care, and drive cost effective and evidence-based practice.

As part of the diabetes MDT, you will use your clinical knowledge, skills and experience to inform therapeutic decision making, and use your independent prescribing skills to optimise diabetes medication regimens.

You will act as an expert resource and a point of contact for the diabetes MDT, other UCLH staff, and patients requiring information about diabetes medicines.

Through service evaluations and quality improvement projects, you will work to develop and enhance our existing systems and processes in relation to safety, efficiency, and productivity using Epic®, the UCLH Electronic Health Records System.

You will develop, implement and maintain clinical pathways relating to complex regimens or the use high-cost drugs by working collaboratively with local primary care organisations across the NCL ICS geography to improve patients’ care in hospital and community settings.

At St Pancras Rehabilitation Unit there is a strong emphasis on patient-centred assessment, evidence-based care planning, self- management and culturally sensitive care. The SPRU nursing team pride themselves in having a unique role in completing detailed patient-centred assessment, care planning and the implementation of treatment strategies that are based on evidence-based practice. The SPRU nursing team also have a unique role in identifying, assessing and supporting patients with self-administration of medication (SAM).

The unit is consistently achieving Outstanding for the compassion of care that staff provide there. The unit provides rehabilitation services for people requiring:

- Neurological rehabilitation
- Complex rehabilitation following a deterioration in functioning
- Disability management

We are looking for nurses who are kind and caring but also understand the importance of rehabilitation and working with patients to improve their independence. In return for everything you give you will receive, support, development, dedicated supervision and a first class learning experience of all aspects of rehabilitation nursing.

We operate 24 hours a day 7 days a week and you will be expected to work on a rota. You will be part of a multi-disciplinary team of doctors, therapists, social workers and health care assistants all focused on rehabilitating our patients to returning home where possible.

The St Pancras Rehabilitation Unit (SPRU) offers specialist rehabilitation service for patients living within North Central London catchment area. Key rehabilitation pathways include multi-pathology, frailty, stroke and neurology. The Nursing staff play an important role in delivering the ethos of a 24-hour rehabilitation and making every contact count. Diamond Ward (Highgate West) is a 17-bed male acute treatment ward. It specialises in the treatment of patients with severe mental illnesses such as schizophrenia, bipolar affective disorder, depression and personality disorder, with associated challenging behaviours/risks. The focus is in detailed clinical and risk assessment with a view to starting early effective treatment. The clinical lead for the team is Dr Kavinda Gunathillaka, Consultant Psychiatrist. The post would involves working closely with other MDT members such as nursing staff, psychiatrists, OTs and psychologists.

Tasks would include participating in daily MDT meetings and ward rounds (clinical reviews), clerking in new patients, reviewing patients’ mental and physical health, ordering relevant investigations, performing physical examinations, blood tests and ECGs and participating in the out of hours core trainee rota. The foundation doctor would be working under the close supervision of Dr Gunathillaka, his specialty doctor and core trainees.

The post may be based full time in Diamond Ward or may be split between Diamond and another treatment ward in Highgate Mental Health Centre. If the post is split it will involve 3 days per week in Diamond and 2 days per week in another ward. The post has arisen because of a gap due to the post not being filled by a Deanery foundation doctor.

Clinical Practice Optimizing Medicines Use: Ensure safe and appropriate use of medications for maximum benefit and minimal risk, regularly reviewing prescriptions, identifying significant drug interactions, and monitoring compliance with applicable laws, including the Medicines Act and Mental Health Act. Pharmaceutical Care at Ward Level: Act as the primary pharmacist at ward level, providing clinical guidance to the multidisciplinary teams. You will be responsible for completing medication histories, formulating pharmaceutical care plans, and monitoring patient outcomes to ensure medicines are effective and safely managed. Patient Discharge Planning: Play a vital role in discharge planning to facilitate timely and accurate dispensing and ensure medicines are labeled and prepared for safe patient use post-discharge.
