Job Description/Scope of Practice
The incumbent will have knowledge of the processes for Reusable Medical Equipment (RME) as required by this position. The incumbent will be required to maintain current knowledge of the Standard Operating Procedures (SOPs) which document the processes and procedures for cleaning, disinfection, sterilization and preparation of RME appropriate to this position. The incumbent will be able to demonstrate their ability to utilize an acceptable proficiency in the use of RME appropriate to this position.

This is an advance performance level position in Sterile Processing Service as a Lead Medical Supply Technician (Sterile Processing Technician). In this position, the incumbent will provide sterilization, preparation, supply items and reusable medical equipment decontamination and distribution support for the medical center. The term "supply item" as used in this functional statement refers to disposable and reusable items for patient care including instrumentation (scissors, forceps, retractors, etc.), reusable medical equipment (suction machines infusion pumps, etc.), surgical trays and sets and various other items that could be processed or assembled through the Preparation and Decontamination areas of Sterile Processing Service.

This is a full performance level MST position. The Lead works in support of the Sterile Processing Service (SPS) management team. Distributes and prioritizes workload among employees in accordance with established workflow and/ or job specializations. Assures an even workflow and distribution of the workload. Revises work schedules to meet anticipated and unanticipated changes in the workload. Assigns work to staff based on experience and training needs. Monitors and reports on the status of work. Ensures SOPs and SPS mandates are followed during the performance of workflow. Reviews work in progress or spot checks work not requiring review to ensure completed work meets supervisors' instructions on such things as work sequence, procedures, methods, and deadlines. Assesses the quality and quantity of work.by reviewing the cleaning, reprocessing, and distribution of technical medical equipment, material and instrumentation. Provides information to management officials concerning performance issues, assignment changes and task completion. Instructs employees on work-related activities, policies, procedures and goals.

Major duties include but are not limited to:

• Responsible for duties related to the direct supervision of all Supervisory MSAs at the Main facility and CBOCs; second line supervision to MSAs of any grade in the assigned areas of: Customer Service, Scheduling, Insurance Card Buffer (ICB) Capture, Recall Reminder, Mission Act referrals, Electronic Wait List (EWL) management and Return to Clinic Orders management.
• Acts as liaison between MSA staff and clinical staff in the outpatient clinics, Veterans and the public.
• Oversees the daily operation of the outpatient clinics, ensures VHA directives and policies are being followed; provides feedback of performance standards; and evaluates work performance of subordinates.
• Instructs employees in assigned tasks and job techniques, conducts formal and informal on-the-job training and answers questions on procedures, policies, and directives.
• Identifies training needs and finds ways to improve production or increase the quality of work within the assigned areas.
• Coordinates the leave of Supervisory MSA's, approving time and leave as appropriated and arranging for or providing cross-coverage in order to provide adequate coverage within the Centralized Scheduling Unit within the Patient Administration section of the Business Office.
• Develops the need for and maintains an awareness of available resources, and makes recommendations to Business Office Leadership regarding needs, distribution or redistribution of these resources.
• Responsible for data collection, validation, interpretation, analysis, and reporting.
• Researches and/or analyzes issues relating to Appointment Management, Insurance Capture Buffer, performance measures, data entry requirements for administrative data, patient care data capture, and electronic workload capture.
• Extract data, using a variety of software packages and data sources, and apply analytical techniques to develop recommendations for process improvement.
• Hears and resolves minor complaints from employees; refers group grievances and more serious unresolved complaints to a higher-level supervisor or manager; effects minor disciplinary measures such as warnings and reprimands and recommends action in more serious cases.
• Interviews candidates for positions in the unit; recommends appointments, promotions, and reassignments to unit positions; identifies and provides for training needs; and finds ways to improve production or increase the quality of work within the area of authority.
• Performs other related duties as assigned.

PURPOSE AND SCOPE:

The professional registered nurse Outpatient RN CAP 1 is an entry level designation into the Clinical Advancement Program (CAP). This position is accountable and responsible for the provision and coordination of clinically competent care including assessment, planning, intervention and evaluation for an assigned group of patients. This may include delegation of appropriate tasks to direct patient care staff including but not limited to RNs, LVN/LPNs and Patient Care Technicians. PRINCIPAL DUTIES AND RESPONSIBILITIES:

• Performs all essential functions under the direction of the Supervisor and with guidance from the Clinical Educator, Preceptor or in collaboration with other Registered Nurses.

SUPERVISION:

Works with the Facility Manager, facility staff and physician to coordinate the facility operations and patient procedures to ensure provision of quality patient care on a daily basis in accordance with policies, procedures and training.  Provides nursing support to patients and staff. Assists with staffing, staff training, equipment, physician and patient relations, cost containment, supply management, medical records, patient billing, OSHA and all company, state and federal compliance. 

PRINCIPAL DUTIES AND RESPONSIBILITIES:

QUALITY:

• Responsible for supporting and driving quality standards through meeting regulatory requirements and the practice of Continuous Quality Improvement (CQI), including use of CQI tools.

PATIENT RELATED:

• Coordinates all aspects of patient care with the appropriate staff members, from admission through discharge.
• May confer with patients prior to the procedure to obtain necessary medical history, discuss procedure and answer questions, as well as, performs nursing assessment on the day of the procedure.
• Ensures patients and patient's families are educated regarding post procedure care of vascular access.
• Acts as a resource for the staff to address patient concerns and questions.
• Ensures patient's procedures are scheduled in a timely manner and the facility capacity is utilized efficiently.
• As needed, assist with patient workflow, including providing direct patient care and monitoring pre, intra and post procedure.
• Identifies and communicates patient related issues to the Facility Manager.
• Oversees and ensures accurate and complete documentation of patient treatments from admission to discharge.
• Confirms written orders, consents and lab work are completed and documented in accordance with facility policies. This includes appropriate preparation of lab requisitions, delivery to appropriate labs, reporting and forwarding of lab results to appropriate physician.
• Assumes primary responsibility in an emergency situation and must maintain competency with all emergency operational procedures, and initiate CPR and emergency measures as needed.
• Administers medications and IV conscious sedation to patients as directed per physician's orders, and in compliance with, company, federal, state and local regulations.

PHYSICIANS:

• Ensures regular and effective communication with all physicians on an as needed basis.
• Attends and participates in Governing Body, assisting with meeting agenda and gathering of data/material for meetings.
• Attends and participates in CQI meetings with physicians, reporting on assigned topics.
• Works with the physicians and staff to ensure possession of all necessary instruments and equipment.

MAINTENANCE/TECHNICAL

• Ensures appropriate operation of facility equipment and technology, including but not limited, patient monitors, defibrillators, medical recording devices and computers.
• Ensures the inspection of all facility equipment on a regular basis.  Report any malfunctions or maintenance issues immediately.
• Ensures the inspection of all facility operating systems, including temperature checks in appropriate areas and inspect the physical structure of the facility.
• Maintains a clean, safe and sanitary environment in the entire vascular access facility at all times.
• Ensures all blood spills are immediately addressed according to company Bloodborne Pathogen Control Policies.

ADMINISTRATIVE:

• Participates in cost control initiatives.  Responsible for efficient utilization of medication, laboratory, inventory, supplies and equipment to achieve supply cost goals following all guidelines established in the formularies.
• Assists in the development and revision of any applicable policies and procedures for the facility as directed to ensure continuous process improvement.
• Initiates, documents and completes ongoing Continuous Quality Improvement (CQI) activities including monthly reports and attend meetings as required.
• Manages medications, including ordering, receiving, and inventory control and checking for expired medications, including appropriate recording of controlled substances as required by law.
• Oversees maintenance of usage logs for all medications with appropriate documentation, as well as, records of drug disposal.  Communicates with the pharmacist to assure medication accuracy, including appropriate recording of controlled substances as required by law. 
• Ensures medical records are accurate, complete and in compliance with policies and procedures prior to discharge of the patient.   Performs chart audits on a regular basis.
• Along with Facility Manager, coordinates the annual medical testing and competency testing for all facility staff.
• Participates in all facility regulatory and accreditation surveys and action plans to address any deficiencies identified.
• Verifies records for all implanted devices are completed accurately, timely and maintained according to records management guidelines.
• Coordinates the collection and forwarding of all vendor invoices to the accounting department.
• Acts as the Facility Manager in the absence of the manager or at the direction of management.
• Ensure nurses' progress notes are accurate and complete.
• Acts as the Patient Safety Officer and conducts quarterly patient safety meetings and fire drills.
• Complies with HIPPA policies and standards regarding patient information and medical records.
• Other duties as assigned.

• Minimum of three years' experience in an Ambulatory Surgical Center/Vascular Access Lab, or a combination of ASC, surgical, Interventional Radiology and ICU/CCU experience.
• Minimum of three years of Interventional Radiology, scrub experience preferred.
• Supervisory or management experience within a medical or vascular access environment preferred.
• Proficiency with the Microsoft office suite is necessary.  Experience with Medical database software.
• Demonstrated management skills necessary to provide leadership and supervision of facility personnel and to ensure the delivery of maximum quality care to all patients.
• Good communication skills - verbal and written.
• Certified in CPR or successfully complete course in CPR certification

Practice Overview

We are a larger Family medicine practice with a Residency Program imbedded into the practice we have 5 core providers within the clinic and 9 residency providers that come through on a rotating schedule. Since we are a Family Practice set in a rural community we see an array of different patients from new born all the way up. We do in house point of care labs such as A1C’s, Hemoglobin, Urine analysis, Microalbumin along with Point of care Strep, Covid, Influenza, and RSV testing, among others. We carry vaccines for both adults and children. We offer telemedicine visits as well.

Essential Functions

PURPOSE AND SCOPE:

The professional registered nurse Outpatient RN CAP 1 is an entry level designation into the Clinical Advancement Program (CAP). This position is accountable and responsible for the provision and coordination of clinically competent care including assessment, planning, intervention and evaluation for an assigned group of patients. PRINCIPAL DUTIES AND RESPONSIBILITIES:

• Administers medications as prescribed or in accordance with approved algorithm(s), and documents appropriate medical justification and effectiveness.
• Initiates or assists with emergency response measures.
• Serves as a resource for health care team, participates in staff training and orientation of new staff as assigned.
• Ensures correct laboratory collection, processing and shipping procedures are performed and reschedules missed or insufficient laboratory collections.
• Identifies expected outcomes, documents and updates the nursing assessment and plan of care for assigned patients through collaboration with the Interdisciplinary Team.

PURPOSE AND SCOPE:

The professional registered nurse Outpatient RN CAP 1 is an entry level designation into the Clinical Advancement Program (CAP). This position is accountable and responsible for the provision and coordination of clinically competent care including assessment, planning, intervention and evaluation for an assigned group of patients. This may include delegation of appropriate tasks to direct patient care staff including but not limited to RNs, LVN/LPNs and Patient Care Technicians. As a member of the End Stage Kidney Disease (ESKD) health care team, this position participates in decision-making, teaching, leadership functions, and quality improvement activities that enhance patient care outcomes and facility operations.

PRINCIPAL DUTIES AND RESPONSIBILITIES:

All duties and responsibilities are expected to be performed in accordance with Fresenius Kidney Care policy, procedures, standards of nursing practice, state and federal regulations.

• Performs all essential functions under the direction of the Supervisor and with guidance from the Clinical Educator, Preceptor or in collaboration with other Registered Nurses.
• Performs ongoing, systematic collection and analysis of patient data pre – during – post hemodialysis treatment for assigned patients and documents in the patient medical record, makes adjustments or modifications to treatment plan as indicated and notifies Team Leader, Charge Nurse, Supervisor or Physician as needed.
• Assesses, collaborates, and documents patient/family’s basic learning needs to provide initial and ongoing education to patients and family.
• Directs and provides, in collaboration with direct and ancillary patient care staff, all aspects of the daily provision of safe and effective delivery of hemodialysis therapy to assigned patients.
• Administers medications as prescribed or in accordance with approved algorithm(s), and documents appropriate medical justification and effectiveness.
• Initiates or assists with emergency response measures.
• Serves as a resource for health care team, participates in staff training and orientation of new staff as assigned.
• Ensures correct laboratory collection, processing and shipping procedures are performed and reschedules missed or insufficient laboratory collections.
• Identifies expected outcomes, documents and updates the nursing assessment and plan of care for assigned patients through collaboration with the Interdisciplinary Team.
• Ensures patient awareness related to transplant and treatment modality options.
• Required to complete CAP requirements to advance.  
• Performs all other duties as assigned by Supervisor.
