1. CLINICAL RESPONSIBILITIES Provision of a high-quality clinical pharmacy service as part of the multidisciplinary team to all patients within the renal unit, to ensure maximum efficacy, safety and economic drug use and to demonstrate professional accountability to enhance patient care. To liaise and work with other specialist pharmacists to share the best practice, to strengthen patient focused pharmaceutical care. 2. RESPONSIBILITY FOR PATIENTS To assist in identifying and managing changes necessary to maintain a high-quality service responsive to patients needs and to consider the implications of changes for other areas/personnel. To assist in the planning and organising of a clinical pharmacy service to renal patients in both the inpatient and outpatient settings. To deal effectively with situations where medication errors have occurred, including liaising directly with patients and staff concerned. Assist in responding to complaints from patients and the public in relation to medicines management issues as required. Ensure that all renal patients are counselled on all aspects of their medication in order to maximise their understanding of their treatment. To deliver a high quality and comprehensive medicines counselling service to patients and/or their relatives on all aspects of their pharmaceutical care including specialist and unlicensed drugs. RESPONSIBILITY FOR POLICY AND SERVICE DEVELOPMENT To assist the Lead Pharmacist, Renal Services in the production and implementation of business plans for the development of the pharmacy service provided to the renal and urology units. Forecasting new drug developments, and identify changes in prescribing practice that may impact on the drugs budget. RESPONSIBILITY FOR LEADING AND MANAGING To share expertise with healthcare professionals on a nationwide level through professional conferences, forum and bodies. The postholder will work with the Digital Leadership team across CLCH, WL and CNWL to develop and enhance and deliver cross-cutting collaboration of digital programmes that support the Trusts strategies and long-term plan commitments. Job Summary: To provide a high-quality, advanced, expert nursing and cost-effective service to patients referred to the Centre for Active Lifestyle Management (CALM) Service. This comprises a multi-professional team (Psychology, Physiotherapy and Nursing) who work with patients with chronic pain to help them improve their quality of life. The Division aims to provide an environment conducive to the on-going development of all staff and advancements in the quality of the service by ensuring provision of a high standard of training, teaching and clinical care. The Clinical Nurse Specialist will be an advanced and experienced practitioner within the field of chronic pain and they will be expected to provide clinical support, working independently or assisting with multi-professional interventions in which full training has been undertaken based on clinical practice standards. The role of Clinical Nurse Specialist requires a high degree of personal and professional autonomy in making clinical judgements. Taking sole and joint responsibility as appropriate for patients during their time in the care of the CALM service to ensure a successful patient-based outcome, which will require assessment, planning and implementation of treatment. The Clinical Nurse Specialist will be required to give advice and guidance to other health professionals regarding chronic pain interventions and clinical practice. They will be responsible for the nursing leadership within the CALM service, giving support and guidance within the multidisciplinary team and contributing to the high-quality care given to patients in accordance with clinical governance. The postholder will ensure the delivery and expansion of an expert and advanced nursing service through effective interdisciplinary care, utilising clinical and managerial expertise. To provide specialist psychological assessments, formulations and interventions for clients in the CYP Psychology Service at the Havens including:o Specialist assessment and clinical formulation of children and adolescents who have experienced sexual assault, rape or who have been sexually exploited. To include psychological tests, self-report measures, rating scales, direct and indirect structured / semi-structured and unstructured observations and interviews with the client, family members and others involved in their care.o Specialist evidence-based, medium term, intervention for children and adolescents traumatised by sexual abuse, monitoring outcomes and modifying / adapting intervention as required and drawing on a range of theoretical options, and supporting their transition to local services where appropriate.
