The main Duties and Responsibilities of this post are: 1.To provide a 7 day community Speech and Language Therapy service to adults who have acquired speech, language, communication and/or swallowing impairments in Haringey. Treatment will take place at home, residential or nursing homes, resource centres and health centres, which may be made on foot, by public transport, bicycle, motorcycle or car. 2. To ensure that all reasonable precautions are taken in evaluating and reducing risk of lone working. 3. To fulfil the role of a Speech and Language Therapist, offering input, in the assessment, rehabilitation and management of adults in the community with complex communication and swallowing difficulties. 4. To ensure that clinical and professional standards of practice are maintained, including ensuring the maintenance of appropriate levels of education and continuing professional development in acquired swallowing and communication disorders, seeking advice where indicated. 5. The post holder will actively participate in regular clinical supervision with a senior Speech and Language Therapist as well as peer supervision. 6. To ensure close and collaborative working with patients, families, carers and members of the MDTs, and to set, monitor and review goals for intervention. 7. To promote the interests of the SLT service and their clients both within the local team and beyond. 8. To contribute to the continued development of clinical and strategic pathways across the ICO to enhance patient journeys and optimise care, working with colleagues in acute and other community and social agencies. 9. To contribute to specific projects in line with the needs of HART and SLT within the community. 10. The post holder will have an understanding of clinical governance, and keep up-to-date with developments in adult community rehabilitation, with special involvement with communication and swallowing difficulties, contributing to audits to evaluate practice, and contributing in research projects. 11. The post holder will develop an awareness of current policies and practice in health care and social services. 12. To contribute to clinical effectiveness by use of evidence based practice and outcome measures. 13. To ensure working practices are supported so that all the disciplines working in the Teams can develop flexible interdisciplinary/multi-agency work practices, to reflect and support productivity initiatives and different working practices when required, which enhance outcomes for clients. 14. On an annual basis, the post holder will be a clinical educator for SLT students on placement and to participate in supporting other AHP students on placement within the team. 15. The post holder will be required to be working at Level C of the RCSLT dysphagia competencies. 16. To undertake any other duties with the post as required in the community, which may include a change in workplace. 17. The post holder will be expected to manage sensitive and challenging situations, in terms of adult safeguarding and mental capacity issues, with support from more senior staff. The post holder will be expected to undertake an advocacy role for clients where necessary. Revalidation and Registration It is the responsibility of all staff registered with a professional body to: Act within the Professional Bodies Code of Practice Maintain their own work profile to ensure revalidation standards are met and assist junior registered staff in achieving revalidation. Contribute and participate in the development of colleagues professional practice across the trust through leading ward and/or department projects, and supporting training. Ensure optimum use is made of working time. This includes ensuring personal and team compliance with all relevant policies, especially hand hygiene, the trust dress code, and MRSA screening policies. Working patterns The Trust is currently exploring ways in which patients can be given more choice about when they can attend appointments at the hospital. In order to make this possible there maybe a future requirement for all staff to carry out patient appointments for patients in the evenings or at weekends. This means that staff may be required to work a shift pattern in future. Shifts will not normally operate beyond 9 pm in the evenings and appropriate pay enhancements will apply. Staff will be consulted about the introduction of / changes to shift systems. Staff working in any department where an on 'call rota' operates will be required to participate in the rota. Managers will discuss with staff the level of 'on call' cover required taking into account their individual circumstances. Staff in nursing posts may be requested to work in any area throughout the Trust by the matron or the site manager.
