CLINICAL/PATIENT/CLIENT CARE: Utilising basic analytical skills that take into account all facets of the childs development, the family and broader social context: Using a range of clinical skills, to assist and at times autonomously lead on assessment and treatment care needs, within a risk assessment framework for children and their families admitted to the Unit. To assist in the planning, implementation and evaluation of family centred nursing care and management plans as agreed with the core team. To support the delivery of a range of daily individual and group structures to promote the therapeutic milieu of the Unit. This includes providing agreed activity groups and individual activities. To observe, communicate and at times, co-work with senior multidisciplinary team members, in the provision of a range of therapeutic approaches on an individual, group or family basis. To provide high quality clear written and verbal accounts of the childs stay in relation to the defined care and management plans. To contribute to evidence-based clinical nursing reports, for parents and multi- disciplinary professionals as agreed with the core team. To actively access and participate in the unit supervision and support process and in line with development, to provide on a shift by shift basis, the same to newly qualified/appointed nurse, support worker and student nurse staff. To follow and inform the development of policies and protocols to support evidence based clinical practice. Proactively manage telephone calls; many may be of a sensitive nature from patients/parents/carers or from other healthcare professions, all of which require a high level of patient confidentiality and sensitivity. Responsibility for providing accurate, up to date, clinical advice relating to queries into the department. MANAGEMENT & LEADERSHIP: Contribute to the promotion of a working culture that improves quality and efficiency within the department. To support the senior management team with the identification of more efficient ways of working within area of responsibility. To comply with all Trust and department policies and procedures. To take charge of defined shifts, supporting the team by organising and managing tasks related to the daily structure and functioning of the service. This includes leading, managing and supporting other qualified support worker and student nurse staff and structures in meeting the care needs of the patient group. To work collaboratively with the core team, taking responsibility for defined tasks, in managing the core team process, maintaining key working relationships with the child, family and multidisciplinary team. To assist the team in the ongoing provision of complex daily routines and structures in the context of a safe, healthy and therapeutic environment. COMMUNICATION SKILLS: Communicate professionally and effectively with patients/parents/carers, the general public and all internal and external agencies both verbally and non-verbally in a timely manner. PLANNING AND ORGANISATIONAL SKILLS: To plan and organise the day to day activity of the shift in the role of Nurse In Charge (NIC) and ensure key information is effectively communicated to ensure appropriate management of future shifts. To plan for/provide reports for defined cases in line with CPA/Clinical review process. When required, assist in the development and delivery of training programmes for staff/students. To support the delivery of a high quality patient care by working closely with the unit team to meet national and locally agreed targets for in-patient CAMHS. PHYSICAL SKILLS AND DEXTERITY: Required to actively participate and at times lead on defined physical and occupational activities on an individual and group basis. Required to effectively and safely utilise a range of approaches in managing challenging behaviours which may include the use of restrictive practices in line with clinical presentation/risk for which the post holder is trained in. JOB DESCRIPTION Post Title: Practice Pharmacist Working Hours: TBC Reports to: Practice Manager & Partner (Medicines Management Lead) Staff management To provide clinical supervision and professional leadership to the medicines management staff. To be responsible for training medicines management staff to perform clinical audits around key areas of responsibility. Patient services To provide medication review services to patients via clinics in the practice, domiciliary visits and in residential and nursing homes, and to deliver pharmaceutical care plans that maximise cost-effective prescribing and improve the quality of patient care, ensuring safe, effective, rational use of medicines To produce patient information leaflets and posters and run medicine awareness projects throughout the year. Assist Partners with the appropriate monitoring and management of their prescribing budgets. To prepare evidence based resources and information to support the medicine management team and all other relevant health professionals in the implementation of rational cost-effective prescribing. To help plan, develop and support the introduction of new working processes within the practice to optimise the quality of prescribing. To implement and manage the repeat dispensing system for the practice. To update and advise of all new processes for the implementation of the repeat dispensing process to all relevant staff. This includes developing, implementing paper prescribing systems within the practice using SystmOne including reauthorising repeat prescriptions as a non-medical independent prescriber. Managing repeat prescribing reauthorisation procedures by reviewing requests for repeat prescriptions and medicines reaching their review date To review the discharge summaries received into the practice. This includes reconciling medicines following hospital discharge and working with patients and community pharmacists to ensure patients receive the medications they need following discharge and records updated accordingly on clinical system Managing medicines queries from patients and healthcare professionals. Implementing drug alerts and withdrawals, for example, MHRA alerts Undertaking Medicines Use Reviews. To support medication review processes, including the identification and management of problematic polypharmacy Responsibility for administration To provide regular support and feedback to practice on prescribing action plans To produce quarterly reports on practice progress to the lead GP for medicines management To update and maintain accurate patient medication records on the practices clinical computer systems including advice given and action taken. To advise the primary health care team on the safe and secure handling of controlled drugs and other medicines, ensuring compliance with medicines legislation. We aim to stop the spiral of debt and mental health issues by providing online information and advice and casework provided by telephone. The service launched in November 2017, commissioned by Mental Health UK, delivered by Adferiad in Wales, Rethink in England, Support in Mind in Scotland, and Mindwise in Northern Ireland. Job Purpose: You will support people with mental illness and their carers, partner agencies with money matters. You will be the first point of contact to Mental Health and Money Advice (MHMA) service. You will triage referrals and: Provide unregulated advice and casework to people (i.e. provide them with financial capability and simple benefits advice) Refer people to the specialist Mental Health and Money Advice Advisers for more complex advice, including regulated debt advice. Signpost/refer to other forms of money support when the persons mental health needs do not warrant the intervention of MHMA. You will also support the Mental Health and Money Advisers with their casework for example supporting with correspondence.
