Mental Health Practitioner - Mental Health Urgent Care Department

Base – Basildon Hospital

Hours – 37.5hrs Full Time (Full Internal Rotation)

The Mental Health Urgent Care Department, is a facility where adults, aged 18 years and above, who are experiencing a mental health crisis can receive an urgent biopsychosocial assessment with immediate care planning. It is a new, purpose built department based at the Basildon Mental Health Unit on the Basildon university Hospital site.

The post holder will work as a Band 6 Senior Practitioner with the urgent care team providing high quality, evidence-based, holistic assessments to patients attending the service. Assessments must be completed in line with your professional code and Trust policies and clinical guidelines

The service offers full emergency mental health assessments and onward care planning with a diverse and highly skilled mental health workforce incorporating support workers, peer coaches, social worker, nurses and doctors.

The role will require the post holder to work collaboratively to support every aspect of emergency mental health care planning from GP referrals, ambulance conveyance to Mental Health Act Assessments, whilst providing 24/7 access for ‘walk in’s.’



You will assess, plan, implement and evaluate care based upon individual service user needs, under the guidance of the Clinical lead as per NICE guidance.

Provide delegated accountability for Band 3 Health Care Assistants ensuring clinical competence and understanding, with effective support and guidance during the shift.

As required by your manager, you will have deputise when required for the day to day co-ordination of the shift within the Urgent Care Department, for example to cover breaks and the clinical lead’s attendance to urgent clinical matters as they arise.

You will work collaboratively with key partner organisations, service users, carers, clinicians and other practitioners within the multi-disciplinary team in delivering services; providing a 7-day a week, 24-hours a day service for 365 days a year working shifts where appropriate and operationally required.

Professional Functions:

• Feedback outcomes of assessments to the department clinical leaders and agree and implement the management plan
• Ensure that patients requiring further assessment and help are referred on appropriately.
• Implement effective joint care planning for people who repeatedly attend throughout the urgent care pathway
• Arrange admission to the mental health services in conjunction with the Urgent Care Department psychiatrist
• Support and participate in the introduction and delivery of short-term psychological interventions for a specific cohort of individuals.
• Liaise effectively with staff in local systems and maintain accurate patient records that provide assessment reports to GP’s and other allied agencies.
• To provide advice and monitor the use and provision of the Mental Health/Capacity Act (1983/2005) and other relevant legislation.
• To provide clinical advice/expertise health education, consultation and staff support to other health professionals
• Have knowledge of how to access appropriate services and develop effective networks and channels of communication
• To actively participate in the achievement of clinical outcome measures and accreditations for the servic

Managerial:
• Support the Clinical Lead in ensuring the effective provision of care, service developments of the urgent Care Team
• In the absence of the Clinical Lead ensure that referral, assessment and workload allocation happens in a timely, flexible and responsive manger, taking into account the wishes of the patients and their carers.
• On a daily basis, support and manage the patients within the urgent care department, providing appropriate prioritisation of the workload within agreed timeframes depending on the nature of the presenting needs and patient safety risks.
• Participate in team meetings, initiating and discussing new methods/models of care using QI methodology and contribute in the introduction of research or trials within the area.
• Participate in the multi-disciplinary and multi-agency decision-making process through team meetings and team building.
• Undertake duties as assigned by senior staff to ensure that all agreed patient quality, safety and operational performance outcomes are delivered within the area of responsibility
• To promote the understanding of urgent mental health care through formal and informal teaching

Suitable applicants will:
• be a qualified RMN .

An exciting opportunity is available for a passionate, ambitious, Band 5 RMN to join our Male Acute Adult Inpatient ward on Cherrydown Ward, based at Basildon Mental Health Unit.

The Acute Inpatient Service provides 24 hour treatment and care in a safe and therapeutic setting, actively engaging and involving people who are experiencing an acute mental health episode which cannot be managed in a less restrictive setting, owing to the degree of risk, clinical need or patient choice. Many of those admitted will require detention under the Mental Health Act 1983 and the Amendment Act 2007. Where necessary, intense supervision and support will be provided.

Patients receive individualised care and treatment based on the Care Programme Approach (CPA) which highlights crisis management and a place of safety, specialised risk assessments and risk management, intensive assessment, treatment and therapy, stabilisation of acute symptoms and relapse prevention.

The service offers:
• Assistance to acquire and enhance coping skills that promote and enable self- help and the self-management of mental health problems and difficulties

KEY RESPONSIBILITIES

Clinical Duties:
1. To assess clients referred to the service.
2. To undertake specialist ADL assessments when required.
3. To make decisions on the level of treatment intervention required and undertake treatment planning; communicating and negotiating treatment plans and clinical reasoning for intervention with clients and their carers.
4. To communicate agreed treatment/care plans to multi-disciplinary team colleagues and partnership agencies and liaise with them at formal and informal meetings.

6. To liaise with in-patient and community based services to ensure smooth transfer of care through the services.

8. To make clinically informed decisions in order to delegate aspects of client care to assistants, junior staff, students, carers and statutory or non-statutory bodies.
9. To regularly review client’s progress in order to evaluate and interpret effectiveness of treatment plans, and amend treatment or discharge clients accordingly.
10. To ensure that treatment offered is based on the best available evidence for effectiveness in the specialist area where it is applied.
11. To develop and initiate interventions based on the integration of theoretical knowledge in order to meet unique client needs.
12. To keep up to date clinical records in line with professional standards and local record keeping policies.
13. To provide written and verbal reports as required at any stage in the treatment process in line with Occupational Therapy Clinical Governance Standards.

Indirect Clinical Duties:
1. To attend clinical case discussions at meetings or ward rounds.
2. To participate in clinical audit, research and evidence based practice.
3. To undertake day to day clinical prioritisation and work planning for small teams.
4. To identify clinical service needs and initiate projects, which could be developed in order to meet challenges.
5. To promote the roles and skills of Occupational Therapy to other professionals, students, clients and carers.
6. To contribute constructively to multi-disciplinary team working.

Profession Specific Duties:
1. To attend professional Occupational Therapy meetings as required.
2. To take responsibility for developing as a professional.
3. To maintain a continuing professional development portfolio.
4. To undertake annual appraisal.
5. To contribute constructively to the supervision process.
6. To provide supervision to junior staff and students.
7. To occasionally represent the Occupational Therapy profession at service development forums.
8. To comply with policies and agreed procedures for annual leave, sickness and time owing.
9. To complete daily diary sheets for statistical purposes.

The Adult Community Psychological Service provides a Tier 2 service to the South Essex locality. The post holder will be expected to provide specialist psychological input to the local community mental health teams comprising the First Response Team (FRT) and Recovery and Wellbeing Team (RWT). The FRT delivers care and interventions to clients newly referred to secondary care mental health services for a period of 6 months. The RWT delivers care plans aligned to recovery focused outcomes to clients with longer term needs. As a member of the multidisciplinary team you will also be required to provide consultation, supervision and teaching to other professionals. The post holder will integrate working with Talking therapies, in-patient, specialist team, and the MDT colleagues.

The role involves developing psychology interventions and an associated strategy for service delivery for working with clients who present risks to themselves and/or others and who have histories of trauma and abuse.They will also be involved in supervising Trainee Psychologists as well as Clinical Associate Psychologists (CAPS), which the Psychological Services Directorate has taken an active role in facilitating the recruitment and training of.The role also involvesoffering advice and consultation on client’s psychological care to non-psychologist colleagues and to non-professional carers, working autonomously within professional guidelines and the overall framework of the team’s and Trust’s policies and procedures.

Hourly Rates (dependent on experience):

*this is inclusive of the 12.5% uplift*

Day: £16.39 to £19.95 per hour

Night/Saturday: £21.31 to £25.94 per hour

Sunday/Bank Holiday: £26.22 to £31.92 per hour

Working on the EPUT bank allows you to gain experience and skills whilst also having the flexibility to work the shifts and placements that fit around you and your commitments.

The post holder will work under the overall supervision of a Band 6 or 7 Supervisor; however, the post holder will be required to work without direct supervision. In addition, following a period of Preceptorship and agreement with the Ward Manager, the post holder will at times take the lead of junior staff, and will be designated to take charge of the ward on a regular basis.

Additionally, responsibility will be held for the development of:
• Assessments, Implementation and evaluation of inpatient treatment plans
• Accurate and timely risk assessments
• Supervision of junior staff
• Providing training for qualified and unqualified staff, including basic and post basic students
• All of the above will be an integral part of the role.
