The Paediatric Emergency Department at Newham University Hospital sees a wide variety of paediatric medical and surgical presentations with over 40,000 children attending the department each year. The post of Paediatric ED fellow is aimed at doctors with some experience of working in either (or both) Emergency Medicine or Paediatrics and is ideal for those wanting to consolidate their skills, confidence and knowledge of the assessment and management of sick and injured children whilst learning how to manage a busy department. Our Emergency Department team consists of Emergency Medicine consultants and trainees, Paediatric Emergency Medicine Consultants, Paediatric trained nurses, Advanced Clinical Practitioners and Emergency Department Assistants and Health Care Assistants. There is a Consultant presence in the department 16 hours per day with 24-hour Middle Grade support, and close working with the Paediatric team.

The post holder will undertaking clinical duties as a Registrar in Paediatric Emergency Medicine at NUH including pro-rata out of hours work. They will work in the Paediatric Emergency Department including the Paediatric Resuscitation Area and Paediatric CDU.

Each post will have a named educational and clinical supervisor.
There will be formal educational oversight of the role, with an induction, educational agreement, a personal development plan and regular appraisal.

In your clinical time you will be based in the Paediatric Emergency Department at Newham University Hospital working as a Paediatric Emergency Medicine Registrar, alongside trainees and specialist doctors.

After an induction period, there will be opportunity to become involved in department quality improvement projects, education, and governance (2-8 hours per week, depending on the project).

We are looking for candidates with previous experience working in Emergency Medicine (EM) or Paediatrics who are able to supervise junior colleagues in the management of paediatric patients presenting to the Emergency Department (ED) and are wanting to further develop their own skills and expertise in Paediatric Emergency Medicine (PEM). The department has four PEM sub-specialty trained EM Consultants who work across both the adult and paediatric EDs. There is an EM consultant on-site from 8am to midnight 7 days a week. Outside these hours, the Emergency Department team is led by a senior EM trainee (ST4+) or trust grade doctor in close liaison with the senior nursing team. There is a close working relationship with the Paediatric team.

We are currently looking to appoint a middle grade / trust doctor to join our team in the department of Breast Surgery at St Bartholomew’s Hospital. This post is to commence as soon as possible and will run for a period of 6 months with the opportunity for it to be extended. We will consider applications from candidates who are aiming for a specialist training number who would like more experience at this grade before national applicaton and also from candidates who are approaching / completed specilaist training either in the UK or overseas who wish to have further exposure to complex breast surgical proceedures and treatment planning in a large, busy, tertiary referral centre in the heart of London. Previous experience in breast surgery is required (to include one stop and basic treatment planning).

Barts Breast Unit is a tertiary referral centre for the treatment of breast disease and is part of Barts Health NHS Trust. The unit is located at St Bartholomew’s Hospital in the City of London which is a large state of the art cancer and cardiac centre. Outpatient clinics are undertaken in a purpose built unit with adjacent imaging and biopsy facilities. We offer the full range of breast procedures including oncoplastic breast conservation and mastectomy with immediate – and delayed – reconstruction. We are able to offer implant and LD flap reconstruction on site and cross site free-flap reconstruction at the Royal London Hospital.

We are looking for two enthusiastic members to join our dynamic team providing acute general internal medicine services to the people of Newham. This full-time fixed term IMT 1-2 (SHO) equivalent post will be available from January 2023 for 12 months in the first instance.
This is an exciting opportunity within our Acute Ambulatory Care department at Newham University Hospital, a 52 bedded unit which will offer care for the initial 48 hours of admission. The unit will include facility for 8 monitored beds.

The successful applicant will contribute to the acute medical care for the hospital which serves London Borough of Newham with a catchment population of 260,000. They will generally be based on the ward delivering the daily care of medical inpatients on the unit, with the support of the Speciality consultant, Specialist Registrar and Foundation trainees. They will also be expected to contribute to the hospital on call rota where they will be responsible for the clerking, assessment and management of patients referred to Acute Medicine. The successful applicants will also have the opportunity to work within ambulatory care working within a supported clinic environment. Duties will be similar to ST1-2 working under the guidance of the obstetrics and Gynaecology Consultant.

The postholder will be covering a variable range of duties in both Obstetrics and Gynaecology. These include acute obstetric cover, acute gynaecology cover, obstetric and gynaecology ward rounds, ward cover, elective obstetric theatre and gynaecology theatre, elective antenatal and gynaecology clinics, and Maternity triage/ Maternity Assessment Unit. Presence in the hospital is required for all shifts.

The rolling rota will be provided prior to commencement of the post. A weekly rota is sent out each Friday, detailing daily duties.

Standard working days 0800-1730
Long Days 0800-2030
Nights 2000-0830


The post holder will be working under the guidance of a registrar at all hours. They are expected to attend and manage patients in the antenatal and gynaecology outpatient clinic. They are expected to attend operating lists, and manage the patients post operatively under the guidance of the consultant.

Consultant cover is on-site between 0800-2200. Between 2200-0800, the consultant covering is off-site. As such, the candidate would be expected to facilaitate primary management for all O+G emergencies overnight under the guidance of a registarar- and understand when to call for consultant assistance in timely manner.

The College Tutor for Obstetrics & Gynaecology is currently Ms Saira Sundar, who is available to trainees for educational and careers advice. The postholder will be assigned an educational and clinical supervisor and will be appraised at 4 monthly intervals, and is required to engage with the Barts Health appraisal system – PReP.




Training:

This post is not part of the Deanery Training scheme, however regular sessions for continuing professional development are provided.
Attendance is expected at daily departmental teaching 0800-0845 (unless scheduled for acute duties).
As per the teaching rota,doctors are expected to present cases for local learning at Risk Management meetings. Doctors are also required to attend PROMPT obstetric simulation training, and a CTG/ Human Factors training day during their rotation.
Deanery trainees, non-training post junior doctors and Medical students are placed within the department, and all staff are expected to support their training.


Leave

All leave should be requested with 6 weeks’ notice, on Health Roster and by emailing the Rota coordinator.


Conditions

All doctors should be aware of current local recommendations regarding vaccination and are requested to have hepatitis vaccinations available via the Occupational Health Department at Newham University Hospital.

Termination of pregnancy is carried out within the unit; however no staff member is compelled to directly participate in the administration of a procedure which results in the termination of a pregnancy if they have a conscientious objection, except in the case of emergency care (when all doctors must provide care/ act in the best interest of a patient).

There is no statutory right to conscientious objection in the case of emergency hormonal contraception as this is not an abortifacient, but concerns should be discussed with the supervising consultant.


Maintaining medical excellence

The ENT service provides a comprehensive range of emergency and elective services to the local community in addition to acting as a referral centre for more complex procedures and tertiary referrals. The service has excellent links with Audiology, Speech and Language Therapy and Oncology. We are looking for an enthusiastic and motivated doctor to join our team in providing front door frailty services for the population of the London Borough of Newham.

You will be based in the emergency department, providing comprehensive geriatric assessment to frail older patients as part of a multi-disciplinary team including emergency department practitioners, therapists and nurses.

This role provides good experience in the management of elderly and frail patientswith theopportunity to develop skills in comprehensive geriatric assessment, deprescribing and communication. There is the opportunity to work with more autonomy and to develop discharge planning knowledge, as well as experience in receiving and vetting referrals. The work is rewarding and has an impact on improving patient care and avoiding admissions.

The role provides a variation from ward work and offers clinic time, audit and QIP opportunities. It would suit a candidate with geriatric medicine experience looking to develop their skills further or those considering geriatric medicine as a long term career option.

The working hours are Mon- Fri 9-5 and the post does not include a fixed on call commitment.

Inpatient COE services:

· Thistle AAU – Acute COE with predicted LOS < 24 hrs (13 beds).

· Thistle Short Stay – Acute COE with predicted LOS 72hrs (24 beds).

· Custom House Ward – Complex COE (20 beds)

· Heather Ward – Acute Stroke Unit (13 beds). Highly performing ASU works alongside RLH HASU regional thrombectomy centre.

Each consultant is responsible on average for 13-16 inpatients, which is variable depending on season.

Outpatient COE services:

We provide a wide range of outpatient clinics. These include general COE, diagnostic memory clinic, falls clinic, stroke and TIA and complex CGA. There are daily HOT clinic slots incorporated into all clinics.

Expected new to follow up ratio in clinic of 3 new to 5-6 follow up patients.

Other COE services provided:

Frailty– We have embedded frailty screening within the Emergency Department for early recognition of frailty. Our front door frailty service has been piloted in the ED over the past six months with excellent results. This post will support us to continue to expand our frailty service.
