Care coordinators play an important role within a PCN to proactively identify and work with people, including the frail/elderly and those with long-term conditions, to provide coordination and navigation of care and support across health and care services. They work closely with GPs and practice teams to manage a caseload of patients, acting as a central point of contact to ensure appropriate support is made available to them and their carers; supporting them to understand and manage their condition and ensuring their changing needs are addressed. This is achieved by bringing together all the information about a persons identified care and support needs and exploring options to meet these within a single personalised care and support plan, based on what matters to the person. Care coordinators review patients needs and help them access the services and support they require to understand and manage their own health and wellbeing, referring to social prescribing link workers, health and wellbeing coaches, and other professionals where appropriate. Care coordinators could potentially provide time, capacity and expertise to support people in preparing for or following-up clinical conversations they have with primary care professionals to enable them to be actively involved in managing their care and supported to make choices that are right for them. Their aim is to help people improve their quality of life. The successful candidate will be based in a local cluster of General Practices as part of the Primary Care Network (PCN). They will be caring, dedicated, reliable and person-focussed and enjoy working with a wide range of people. They will have good written and verbal communication skills and strong organisational and time management skills. They will be highly motivated and proactive with a flexible attitude, keen to work and learn as part of a team and committed to providing people, their families and carers with high quality support. We are looking for associate/casual Instructors to join our team within Benmore Centre and our other associated local buildings.

We are focused on providing high quality Outdoor Learning to the school children of Edinburgh City along with a wide mix of other local authorities, youth, charity, corporate and Duke of Edinburgh related organisations. Our course delivery aspires to contribute to each school’s Curriculum for Excellence outcomes through Outdoor Learning.

This is an opportunity for a casual cook to work on a relief basis at Benmore Outdoor centre, Dunoon.

Our Outdoor Learning Centre caters for school residentials during term time and commercial bookings and events during school holidays and weekends.

Shifts will be offered on a casual/relief basis to cover holidays, absences and events.

You will be responsible for food preparation through to serving. High standards of food safety and hygiene are essential. The successful applicant will be a skilled cook, flexible and organised. Able to work efficiently and safely within company policy and industry standards.

You will cater for groups of 20-120 , preparing, cooking and serving meals from breakfast, lunch and dinner. Shifts may involve all or some of these meal services.

Earliest start time would usually be 7.30am and latest finish time would normally be 7.30pm. Willingness to work weekends would be desirable.

To support the day to day operation of a licensed gaming venue. Welcoming customers, keeping the venue
clean and tidy and carrying out any minor repairs across the machines, while reporting more serious ones.
Handling any customer concerns and keeping the venue legal by challenging anyone under 25. Cash handling,
while being responsible for floats daily. Offering customers promotions.
This role will support the MSA engineer in maintaining standards and driving sales and covering holidays.

Working locally, you will join one of our teams delivering great quality individualised support to adults who have a variety of support needs, where every day is different in this challenging but rewarding role.

Applicants must be prepared to work flexibly as part of the team to meet the needs of the people who use our service. Supports will include work in the evenings and weekends as well as sleepovers and waking nights when required.

Access to a car would be an advantage but not a requirement for the post.

You will liaise with our Home Manager, Nutritional Therapist and staff to ensure high-quality fresh, home-cooked food is provided every day. You will also establish a rapport with our residents and their families in order to satisfy their dietary needs. You will be responsible for:

Preparing, cooking and serving meals for residents and others as required, in a timely manner.
Providing a varied diet, catering for the cultural preferences of those who use our services.
Catering for celebrations and other events in our Home.
Ordering supplies within the catering budget and company policy guidelines, using approved suppliers in the absence of the senior cook
Maintaining accurate catering records in accordance with HACCP.
Participating in inspections by Environmental Health and/or care regulators and Local Authorities.
Maintaining a high standard of hygiene within the kitchen by adhering to HACCP and COSHH guidelines.
Assisting in the maintenance of a safe and healthy working environment by ensuring adherence to Health & Safety and Welfare.
