We are seeking UK NMC Registered Nurses to join our team working at our 20 bedded care home with nursing. Experience in working with adults 18-65 with learning disabilities and health conditions would be an advantage.

Responsibilities and Duties
Our nurses support adults with nursing needs, learning disabilities, and mental ill-health conditions in a nursing home environment and work on a 12-hour shift pattern, mostly night shifts but you may be asked to cover some days. Including supporting with nursing needs, daily living skill development, personal care, and working with some behaviors which may present as challenging. Full induction, mandatory training, and regular support and supervision are provided to include Clinical supervision.
You will be expected to support the Home Manager and assistant manager with all aspects of nursing care from the assessment of need, designing support plans, and delivery of care.

Since vulnerable people need care all the time, we do ask that you work weekends but not all, this ensures our service users always have the support required. This is a full-time role (36-hour contract) with the opportunity of overtime although this is not mandatory. Registration with NMC essential.

As our Maintenance operative, you’ll be a valuable member of our staff, promoting the well-being of our service users and the successful running of our care home.

This role covers our services in Plymouth, South West England.

The ideal candidate will have maintenance experience in a social care and health care setting, and have good literacy and numeracy skills. A qualification in one aspect of maintenance (e.g., plumbing), would be preferred, but not essential as full training given. The ideal candidate will have a general knowledge of all aspects of household maintenance, and be able to identify when an outside contractor is required and advise accordingly.

This is a full-time contract Monday – Friday with some element of emergency on-call expected to include paid call-outs if necessary.

You will be working jointly with the Home Managers to ensure that our homes are maintained to the highest of standards. Your responsibilities will include -

Liaise with home manager to prioritise and complete maintenance tasks.
General care home maintenance (moving of furniture, mowing/weeding, clearing drains and soil pipes, painting and decorating, and hanging pictures. Mending leaks. Changing taps. Includes ladder work.
Ensure all ventilation units and lights are kept clean. Replace any defective light bulbs/diffusers
Liaise with Home Manager or delegated Staff Member regarding maintenance work to be carried out
Ensure all equipment used by the maintenance operative is clean and well maintained
Checking control panels and electrical wiring to identify issues
Perform minor fixes such as repairing broken locks, filling gaps on walls etc.
Install appliances and equipment
Check functionality of safety systems
Inspect and troubleshoot equipment and systems
Record keeping for tasks identified and completed
Manage a small maintenance budget and keep records of expenditure

Key Tasks and Responsibilities of the Post Clinical To undertake client interventions and reviews by CCRT workers, regularly meeting the relevant professionals to discuss progress, and reporting back if any problems occur. PRIMARY DUTIES & AREAS OF RESPONSIBILITY Clinical To manage a specialist caseload of patients referred to the Enhanced Supportive Care service within the Trust and contributes to the multidisciplinary team. To act as a clinical resource, providing specialist knowledge to members of the dietetic department on Nutrition Support. To provide expert advice to patients on an inpatient or outpatient basis as required, and by telephone contact if appropriate to review management. To attend weekly MDT meetings. Undertake nutritional assessment, dietary review (where appropriate) and design nutrition care plans taking into consideration anthropometry, biochemistry, pharmacology, and medical history. To provide a source of expert knowledge and advice on nutrition in palliative oncology, and review patients nutritional status and adapt care plans accordingly. To maintain accurate and timely dietetic record in the patient records in line with national and local record keeping standards. Monitor the delivery of services as advised by the Line Manager or Head of Department. To work co-operatively with other professionals to ensure delivery of coordinated multidisciplinary service to patients requiring nutrition support. Maintain good communication with all agencies involved with patient care e.g. G.Ps, consultants, nurses, pharmacy, allied health professionals, catering etc. Communicate effectively with patients and carers to maximise disease management. What do we look for in a Care Worker? 

• Certicate III or above in Aged Care or Individual Support, Disability or Nursing.

The role of Partner Peer Support Worker has been developed specifically for people who have lived experience and who may of care for (or have cared for) people with mental health challenges. Through sharing wisdom from their own experiences, Peer Support Workers will inspire hope and belief in recovery and supporting others to gain a sense of control over their lives. As a highly valued member of the multi-disciplinary team, the Peer Support Worker will provide peer support and practice assistance to service users in order for them to regain control over their lives and build connections with their local communities. Based with the Perinatal Team the Peer Support Worker will work across the Perinatal and Maternal Mental Health Service pathway, and third sector partner agencies, supporting service users transition between services. The Peer Support Worker will act as a recovery advocate across mental health service pathways and act as ambassador of recovery for the trust with external agencies and partner organisations. The Peer Support Worker will be expected to be involved in the on-going development of the role. Responsibilities
Provide personalized care and support to clients in their daily activities
Assist clients with personal hygiene, dressing, and meal preparation
Encourage and promote independence by helping clients learn new skills
Develop and maintain positive relationships with clients, families, and colleagues
Document and report any changes in client behavior or condition
Participate in team meetings and contribute to care planning
Help clients access community resources and recreational activities
Requirements
Relevant experience in a support or care role
Strong communication and interpersonal skills
Ability to work as part of a team and independently
Understanding of person-centered care principles
Empathy and a genuine desire to help others
Flexibility to work varied shifts, including evenings and weekends
Basic understanding of safeguarding and health and safety regulations
