This 9 to 5 role will have a special emphasis around improving access, flow and discharged from the West London Crisis Recovery House (WLCRH). The post entails taking responsibility for assessment and management of service users experiencing a crisis that may, without CAHTT intervention require a hospital admission. These referrals will be coming from a variety of sources (LPS, Inpatient Wards, Community Teams and other CAHT Teams & SPA). You will be dealing with these referrals that have very specific response times and liaising with relevant professionals. You will also have significant managerial & operational responsibility day to day.

CAHTT is a multidisciplinary team. We work closely with Inpatient Services, SPA, MINT, Early Intervention, Social Services, the Police, Statutory and Voluntary agencies. This would be a really exciting time to join the team as the function of the team is changing and the Band 7 Senior Crisis Care Practitioner role is key to these changes.

Regular monthly supervision and reflective practice will be provided. As a Senior Practitioner within the team you will be working shift work 37.5 Hours a week. The post holder should have a variety of significant experience and previous community experience would be ideal.

The role:

• Providing excellent effective care and support to adults with profound learning disability and complex needs
• Assist the RM in general care/health care administration matters within the service, including cover for staff holidays/other absences, individual care plans
• Promote independent living skills and working in a person -centred way
• Action and work with all care and support plans tailored to the needs of each individual.
• Support with personal care needs where required and ensure that staff are trained and have the support needed to complete their tasks.

About You:

• You will be a positive, proactive and passionate person who has the desire to want to support vulnerable adults with learning disability and complex needs.
• You will work in a person -centred way and ideally have experience of working in the social care sector preferably in a similar setting and have senior and supervisory skills
• Level 3 in Health & Social Care
• Your flexible approach to work will be key and be able to work between the hours of 8am -22.00 Monday to Sunday working occasional weekends.
• Ideally you will be a car driver with access to your own vehicle.

In this vital role, you will be the welcoming face of our college, greeting visitors, staff, and students at the main reception desk. Your responsibilities will include operating a busy switchboard and performing a variety of administrative tasks to support our reception team.

Main Responsibilities

Additionally, you'll need to have knowledge of regulatory requirements and best practice in line with CQC/CIW guidance and experience of working within the care sector at management level. You'll also need to have a full UK driving licence and access to your own vehicle.
