A qualified lawyer with experience in all aspects of community care law/adult social care law and to a lesser extent education law (SEN/School queries).

Will take up some ongoing caseload and fresh queries in these fields without any specific training.

Hours: 36 hours per week

Salary: £14.50 per hour

ABOUT THE ROLE

As a Senior Care Assistant in a care home, you’ll look after the different needs of our clients' residents to enable us to deliver quality, person-centred care and support. Your role will involve supervising and inspiring a team of dedicated Care Assistants alongside the Deputy General Manager. You can also expect to support the clinical team in a specific area, such as medication, assessments, care plans or care reviews. Each department will have their own priorities on a day to day basis. Reception: Support the Reception Team Leader and Deputy Team Leader in a variety of roles as required. Provide the first point of contact for patients and visitors to the practice in a professional manner. Handle requests for appointments in line with practice protocol to ensure the efficient running of the practice appointment system. Handle home visit requests in line with practice protocol. Handle patient queries and enquiries efficiently, both face to face and on the telephone Record and communicate messages for other members of the Practice Team and other agencies accurately and without delay. To be the main contact point for non-clinical medication related queries from pharmacies, nursing homes, and other health care professionals, to improve the accuracy of patient medication records. To deal with and process requests for repeat prescriptions in an accurate and efficient manner. Respond to requests for test results from patients and give results where authorise, whilst strictly adhering to guidelines on patient confidentiality. Maintain accurate computerised patient records, register new patients on to clinical system, amend registration details and input clinical data in line with practice protocol. Deal with all appropriate tasks and items in generated throughout various computerised platforms such as EMIS WEB, Klinik Online Traiage and NHSMail. Senior Triage Clincian (RGN)

Opportunity available for a Registered General Nurse to work within the WARD Based , Based in Hereford and Surrounding Areas.

The team sits within the Ward Based team with the role predominately providing and undertaking a wide range of physical health assessments and to support colleagues in undertaking physical health assessments. You will work collaboratively with primary health, develop and maintain links with GP practices and establish strong working relationships with other specialist services and signpost service users as required. As part of this role you to have accountability and responsibility for implementation of physical health pathways which offers physical health monitoring and intervention alongside medication administration. You will be providing service user groups to further assist in the process of promoting physical well-being of patients you will work as a specialist across the treatment and assessment interventions, overseeing the process of implementing physical health strategies and undertaking supportive roles in complex cases where physical health needs are identified. It is essential you have a Valid Driving License and access to a vehicle.

This role is to commence asap with the shift patterns being Monday to Friday

The Hourly Rate for this role is: £26 Per Hour
