As a Care Assistant for our Emergency Care Scheme, you will provide care to service users living in their own homes with family members, that is flexible and responsive to their needs. You will establish and maintain effective systems for local social work practice governance, quality and outcomes to ensure the best outcomes for service users. Is the champion for professional social work practices within adults, leading on the development and review of policies and procedures relating to the delivery of social work. Bridges the gap between professional and managerial responsibility, to influence the delivery and development of social work practice. Takes a lead role in ensuring the framework for practice, quality, performance and use of resources is operationalised, using practice evidence, performance information and intelligence to identify and address performance/quality variation across teams. Maintains an overview of the effectiveness of adult social work practice and represents the views of practitioners to all levels of management and Elected members in Kirklees and to partner agencies. Leads on audit activities to ensure agreed objectives and targets are met. Develops and sustains continual quality improvement and best practice. Ensures adult social work employees are supported with regular guidance, mentoring and coaching to enhance the quality of practice. Ensures the Council is taking an active role in regional and national networks, including liaison with the Chief Social Worker, contributing to developments and sharing best practice.

Attendance Support Officer Grade 6, Points 13-20 37 hours a week Term time only plus additional 5 days by agreement with line manager Equivalent to 39 weeks Salary- £26,873-£30,296 full time (actual £23,089-£26,030) (Subject to April 2024 pay increment) Temporary for 1 year. (This role requires you to hold a full valid driving licence) Chosen Hill School is looking for someone to identify and support students with low attendance and who need to improve their attendance. The successful applicant will provide support and challenge to parents in line with our policy on attendance. You will work collaboratively with the Education Welfare Office (EWO), Pupil Premium Manager and Attendance Administrator liaising with agencies as well as develop and maintain links between school and the homes of students. You will meet with students regularly to identify barriers to attendance. The ultimate aim of the role is to remove these barriers to build good attendance identified students.
The role will involve working with students in Year 7-11.

Our Field Support Supervisors will be responsible for managing a team of colleagues with the purpose of helping people to gain more independence in their daily lives. You could be developing and implementing person centred support plans, conducting care needs assessments, inducting new colleagues, completing supervisions/appraisals, completing medication competencies and medication audits, attending mutli-disciplinary meetings, completing reports and generally assisting the Branch Manager in the day-to-day management of our services. You’ll never lose sight of why you choose to do this, as our Field Support Supervisors regularly visit their services and spend time with the people we support.

You will also take part in the on-call rota (although this may be subject to change depending on the needs of the service).
