An exciting opportunity has arisen for an Information Analyst within the Information Management & Business Intelligence function at Cheshire and Wirral Partnership.

The role is a developmental one where we are looking for someone with experience within data analytics to be part of a team responsible for the production of Internal and External performance dashboards, reports and the submission of data to external sources such as NHS England, Cheshire and Merseyside Integrated Care Board (ICB) and other Commissioning bodies as well as supporting Care Group analysts when required.

Ensuring rotation of foodstuffs, ordering fresh food and other food, monitoring and recording temperature checks.

As Assistant Cook you will assist in leading the catering team to provide nutritious and enticing meals for the residents in our care. The Paramedic will provide care for the presenting patient from initial history taking and clinical assessment through to the diagnosis, treatment and evaluation of care. They will demonstrate critical thinking in the clinical decision-making process, including assessment and diagnostic skills, leading to the delivery of safe care for all patients. They will work collaboratively with the healthcare team to meet the needs of the patients, supporting the delivery of policy and procedures. The Paramedic will provide a holistic and clinical service, with support from GPs as required, implementing agreed management plans and following approved protocols as appropriate. On a typical day, the Paramedic will be expected to carry out both face to face and telephone triage, there may also be a number of home visits depending on the demand. The role will involve a consultation with patients who present on the day and via booked appointments. The Paramedic may also perform ward rounds at Care Homes, which will include the construction and implementation of individual patient care plans. Mentorship and supervision will be provided by designated senior medical personnel. The level and type of supervision will be dependent on the post holders acquisition of skills and knowledge and determined by the organisations clinical governance arrangements. The practices hold regular education meetings and the Paramedic would be expected to attend and partake in these. Duties and Responsibilities Clinical Participate in all treatment and preventative healthcare services in the Practice as delegated and agreed by the supervising GP(s). Provide direct clinical care to patients using established clinical guidelines. Consult and refer patients to physicians, medical specialists and other health professionals as indicated. Interview patients, take medical histories, perform physical examinations, analyse, diagnose, and explain medical problems during surgery consultations and home visits. Recommend and explain appropriate diagnostic tests and treatment. Request and interpret results of laboratory investigations when necessary. Perform specialised diagnostic physical exams and treatment procedures. Instruct and educate patients in preventative health care. Conduct telephone consultations which may involve discussing the result and implications of laboratory investigations with patients as well as dealing appropriately with routine and emergency complaints Order laboratory tests as required and agreed under supervising doctor(s). Be involved with the triaging requests from patients that arrive by electronic communication. Fully document all aspects of patient care and complete all required paperwork for legal and administrative purposes. Participate fully in the clinicians duty rota including the provision of home visits when appropriate. Formulate diagnoses and treatment plans, in consultation with or referring to Supervising doctor(s) as appropriate. Give clinical instructions to nursing staff and Primary Care Team as required. Have a good understanding of and adhere to current Safeguarding principles and PCN policies and know how to affect these when required in order to act in patients best interests. Training/Educational Development Ensure a Personal Development Plan is in place which shows continuing training and professional development to meet clinical governance guidelines. Keep up to date with relevant medical research, technology and evidence-based medical practice by attending education courses and professional meetings, reading journals etc. Perform a variety of research and analysis tasks associated with improvement of clinical care, medical diagnosis and treatment where appropriate using the following means: Audit of clinical practice. Review of relevant literature. Attend regular educational meetings organised by the practice in order to update clinical knowledge, practice policy and guidelines and disseminate other useful information relevant to the provision of adequate healthcare for patients. Regularly reflect on own practice (and keep a record of learning encounters) in order to identify learning needs and encourage self-directed lifelong learning and continued professional development. Administration Send and receive written information on behalf of the Practice regarding matters of insurance, housing and other issues relating to the physical and social welfare of patients. Work to deliver with other members of the practice and Primary Healthcare Team, the objectives of the GMS contract, including the terms of the Quality and Outcomes Framework and Practice Based Commissioning. Attend regular multi-disciplinary meetings organised by the Practice in order to discuss the health and social needs of particular patients. Other responsibilities Supporting the practice staff and responding to requests for advice and assistance from the practice reception, secretarial and nursing staff. Attend multidisciplinary, clinical and practice meetings. Provide additional enhanced services to the practices that meet the needs of the practices patient population and maximise the income streams to the practice Awareness of and compliance with all relevant practice policies/guidelines, e.g. prescribing, confidentiality, data protection, health and safety, annual QOF Assessment of the Practice. Job Summary
An exciting opportunity has arisen for a qualified and enthusiastic Lead Clinician/Responsible Clinician in Palliative Medicine to join our multidisciplinary team at the Hospice of the Good Shepherd. We seek a professional leader to ensure our medical team operates in line with GMC guidelines and supports our strategic objectives. You will represent the medical team to the Hospice Senior Management Team and embody our values.

Main Duties

You will contribute to the care of patients in our ten-bedded inpatient unit and Living Well Centre, providing medical input for a wide variety of conditions. Your responsibilities will include:

• Overall coordination of medical care for inpatients.
• Medical input into the Living Well Centre.
• Communication with colleagues in the hospital and community.
• Attendance at multidisciplinary meetings.

You will play a vital role in supporting our dedicated team of specialty doctors and contribute to the professional development of staff through teaching, research, and quality improvement activities.

About You

To apply, you will need:

• Full GMC registration with a licence to practise.
• At least 6 years of postgraduate experience, including a minimum of 4 years in palliative medicine or equivalent experience as a Responsible Clinician.
• Experience working within a multidisciplinary team.
• Experience with audit and quality improvement projects in palliative medicine.
