Scope and purpose of the role To deliver a high standard of patient care as a salaried GP in nursing Homes, using advanced autonomous clinical skills, and a broad and in-depth theoretical knowledge base To manage a clinical caseload, dealing with patients needs in a nursing Home setting Provide advanced GP care and case management to patients with multiple complex long-term conditions Undertake medical and social care assessments to initiate interventions to improve quality of life in the nursing Home setting Reduce the fragmentation of care To provide informal training and support to the nursing home team (where applicable) and ensure the highest standards of care are provided for the patients Job Summary The post holder is an experienced GP, who acting within their professional boundaries, will provide care for the patient from initial history taking, clinical assessment, diagnosis, treatment and evaluation of their care. They will demonstrate safe, clinical decision-making and expert care for patients within the general practice. They will work collaboratively with the multi-disciplinary general practice team to meet the needs of patients, supporting the delivery of policy and procedures, and providing clinical leadership as required. Key Responsibilities Clinical role The post-holder will Make professionally autonomous decisions for which he/she is accountable Provide face to face visits via a clinical ward round during the working week. Provide a first point of contact for Nursing Homes for advice regarding medical/nursing conditions of patients, making use of skills in history taking, physical examination, problem-solving and clinical decision-making, to establish a diagnosis and management plan. This includes a comprehensive review (ensuring coding of all significant health conditions and allergies) of newly admitted patients to the Nursing Home and patients readmitted to the Nursing Home following discharge from hospital. This process would include a post-discharge reconciliation of the medication. Instigate necessary invasive and non-invasive diagnostic tests or investigations and interpret findings/reports To prescribe safe, effective and appropriate medication as defined by current legislative framework. This would include the prescription of monthly repeat medication, acute medication and the additional/cessation of medication as indicated (e.g. following discharge from hospital or from the advice of other healthcare professions including but not limited to specialist consultants). Additionally, this includes liaison with both community pharmacists (for example to ensure the continuity of supply of medication) and other pharmacists (including the medicines management pharmacists from GSTT). Provide polypharmacy/drug reviews of medications. To prescribe medication safely and in line with local CCG/national guidance including evidence based and cost-effective prescribing. Provide safe, evidence-based, cost-effective, individualised patient care. This significantly includes documentation about resuscitation and advanced care planning (onto recognised registers such as Co-ordinate My Care) of all patients newly admitted to the Nursing Home as well as a retrospective review of already admitted patients where such resuscitation and advanced care planning discussions have not yet been performed. This is likely to be performed with the patient and/or their representatives (dependent on circumstances including where which involving the patient may not be appropriate) and also be done in Best Interest meetings with other relevant healthcare professionals where a patient lacks capacity about advanced care decision making (and where an advanced directive does not exist). Offer a holistic service to patients and their families, developing where appropriate an on-going plan of care/support, with an emphasis on prevention and self-care Promote health and well-being through the use of health promotion, health education, screening and therapeutic communication skills Refer patients directly to other services/agencies as appropriate including social services and allied agencies/services. Work with nursing, medical and health care assistant colleagues to ensure that National Service Frameworks (e.g. Coronary Heart Disease/Older People/ Diabetes /Mental Health) are being delivered. Work with multi-disciplinary teams within the locality, and across the wider Health Economy, to promote integrated and seamless pathways of care. Extended Day Care Play Assistant required to start as soon as possible. Contracted hours 15 hours per week: 3.00 per day (3pm-6pm), 5 days per week, over 38 weeks. The band 6 senior staff nurse role comes with added responsibility of supporting ant teaching junior staff. CLINICAL To identify, prioritise and manage patients who require occupational therapy input. Including education of the multidisciplinary team (MDT) to promote appropriate referrals for both discharge planning and rehabilitation. To assess and treat patients with complex needs who would benefit from a coordinated multidisciplinary assessment, rehabilitation and discharge services. To be competent in the administration of specific occupational therapy assessments of motor, sensory, cognitive, perceptual, psychosocial and occupational areas. To be competent in the development and implementation of remedial and preventative treatment programmes addressing motor, sensory, cognitive, perceptual, psychological and occupational areas. To provide specialist occupational therapy input to Critical care, including intensive care units and high dependency units at Denmark Hill site. To identify risks, complete risk assessments and implement risk management strategies including the use of therapeutic manual handling techniques and the lone working policy and document this accurately. To recognise any change in the condition of the patient and alter treatment accordingly or escalate to medical teams as required. To work within local policies and procedures, national standards and guidelines and adhere to the Royal College of Occupational Therapy professional standards and ethics. To support patients in setting their own goals, then implementing this and evaluating appropriately. To plan and implement individual and/or group interventions, in collaboration with clients and carers, to achieve therapeutic goals. To select evidence based techniques as part of a coordinated treatment program. To assess and demonstrate to patients and carers the safe use of equipment and adaptations and recommend provision of equipment to support discharge. To liaise with professionals in onward services to ensure continuity of care. To respect the individuality, values, culture and religious diversity of patients and contribute to the provision of a service sensitive to these needs, ensuring equity of service provision. To make appropriate clinical observations and keep accurate electronic records and activity data in accordance with professional and trust standards. To write occupational therapy functional and discharge reports for social services, onward inpatient services and community services. To work as an autonomous practitioner and display appropriate problem solving, analytical and judgement skills. To assist with planning and prioritisation of the team and departments workload in the event of staff absence to ensure the occupational therapy priority services are maintained. To comply with all relevant trust policies, procedures and guidelines, including those relating to equal opportunities, health and safety and confidentiality of information. COMMUNICATION To communicate complex information to patients and family/ carers who may have barriers to understanding. To actively participate in team meetings within the occupational therapy department and with the MDT, including weekly MDT meetings, case conferences and best interest meetings and to delegate for specialist or team lead occupational therapist as required. To be skilled in communicating reasoning and motivating patients to participate in sessions. To communicate professionally in person and by telephone with families/ carers and other professionals. To manage differences of opinions and difficult conversations within the team, MDT and with patients, families/ carers MANAGEMENT AND ADMINISTRATION To supervise and provide support for occupational therapists, occupational therapy assistants and technicians and students. To be aware of the importance of staff well being, accessing trust support as required and raising any concerns with supervisors or senior staff. To contribute to the hospitals clinical governance arrangements and quality agenda. To assist in developing services and standards according to the occupational therapy objectives. To participate in the planning, evaluation and audit of practice, clinical pathways and protocols within the critical care occupational therapy service with a view to measuring efficiency and ensuring high quality standards. To be responsible for competent use and maintenance of equipment, raising any concerns and reporting when further resources are needed. To carry out administrative duties as required e.g. assisting in ordering, receiving and maintaining stock and managing petty cash. To have good IT skills and use a variety of IT systems to input patient and staff data as required. TRAINING, EDUCATION AND RESEARCH To review and reflect on own practice and performance through effective use of professional and operational supervision and appraisal. To promote development of occupational therapists, occupational therapy assistants, technicians and students according to their training and development needs as identified through supervision and appraisal. To regularly supervise occupational therapy students on practice placement. To demonstrate the ability to critically evaluate current research, apply them to practice and disseminate findings at a local level. To broaden research and development skills through participation in local audit and research projects. To demonstrate ongoing personal and professional development through participation in internal and external development opportunities, recording outcomes in a portfolio. To keep in touch with new developments and changing trends in critical care occupational therapy and participate in wider issues of providing an occupational therapy service i.e. quality assurance, public relations including liaison with the community; planning and policy making. To demonstrate leadership skills through the management of designated projects. To participate in and present on the critical care in-service training programmes, multi-disciplinary trainings, study day and external conferences. To undertake any other duties commensurate within the grade.
