The Lead Medical Support Assistant (LMSA) in V16 Clinical Resource Hub is assigned to the Central Arkansas Veterans Healthcare System, Little Rock, Arkansas. The LMSA will normally work at the HUB located on 900 South Shackelford Road, Little Rock, AR or remotely. The LMSA Performs duties related to the receipt, intake and indexing of health and administrative information and will interface with personal computer (PC) applications as well as numerous Vista applications. The Primary Purpose of the Position is: to serve as a psychologist providing co-located collaborative care (CCC) services within primary care Patient Aligned Care Teams (PACT). The Primary Care Mental Health Integration (PCMHI) psychologist participates in PACT and PCMHI team huddles and is available for rapid face to face consultation and coordination of care with primary care staff members. In addition, the PCMHI psychologist provides same day mental health evaluation and triage services to Veterans enrolled in PACT. The PCMHI psychologist provides brief (1-6 visits), problem focused interventions for Veterans experiencing mild to moderate mental health concerns (e.g., mild/moderate depression or anxiety, substance use disorders etc.). The PCMHI psychologist may also provide behavioral health interventions to address common health conditions that present in primary care (insomnia, stress management, chronic pain, weight management) via individual or group behavioral health interventions. The PCMHI psychologist works in collaboration with the PCMHI team to assist the PACT team with follow up on positive behavioral health screens.

Duties include, but are not limited to:

Clinical (100%)

1. Provides initial assessments for patients who are being seen by primary care.
2. Provides therapeutic interventions for patients with mild/moderate MH concerns using a brief, problem focused evidence-based care approach. Issues cover a full spectrum of diagnoses but patients do no and do not generally require specialty mental health care. Services include individual and couples psychotherapy.
3. Provides psychoeducational groups and focal group therapy.
4. Provides consultation to PACT Team staff on a wide variety of patient care issues.
5. Participates in PACT Team huddles and other PACT meetings.

The Primary Purpose of the Position is: to serve as a psychologist providing co-located collaborative care (CCC) services within primary care Patient Aligned Care Teams (PACT). The Primary Care Mental Health Integration (PCMHI) psychologist participates in PACT and PCMHI team huddles and is available for rapid face to face consultation and coordination of care with primary care staff members. In addition, the PCMHI psychologist provides same day mental health evaluation and triage services to Veterans enrolled in PACT. The PCMHI psychologist provides brief (1-6 visits), problem focused interventions for Veterans experiencing mild to moderate mental health concerns (e.g., mild/moderate depression or anxiety, substance use disorders etc.). The PCMHI psychologist may also provide behavioral health interventions to address common health conditions that present in primary care (insomnia, stress management, chronic pain, weight management) via individual or group behavioral health interventions. The PCMHI psychologist works in collaboration with the PCMHI team to assist the PACT team with follow up on positive behavioral health screens.

Duties include, but are not limited to:

Clinical (100%)

1. Provides initial assessments for patients who are being seen by primary care.
2. Provides therapeutic interventions for patients with mild/moderate MH concerns using a brief, problem focused evidence-based care approach. Issues cover a full spectrum of diagnoses but patients do no and do not generally require specialty mental health care. Services include individual and couples psychotherapy.
3. Provides psychoeducational groups and focal group therapy.
4. Provides consultation to PACT Team staff on a wide variety of patient care issues.
5. Participates in PACT Team huddles and other PACT meetings.
6. Participates in PCMHI team meetings.
7. Completes clinical reminders and follow-ups as needed. Is attentive to all Performance Measures for clinical care.
8. Provides supervision of trainees conducting direct clinical care.
9. Facilitates liaisons with other health and mental health care programs in order to effectively coordinate services.
10. Complies with national and local performance measures, including timely health care screening and completion of clinical reminders for all patients.
11. Complies with all CPRS documentation procedures, including timeliness standards for documentation and proper patient check out procedures (e.g., entering diagnostic and procedure codes).
12. Attends and participates in administrative and informational meetings for the Psychology Service and Mental Health Service Line.
13. Completes required trainings in a timely manner.

The Advanced MSA primary duties are to perform a variety of administrative support functions in connection with Veteran's inpatient and outpatient visits, care, and treatment while working collaboratively in an interdisciplinary coordinated care delivery model. Work involves specialized administrative judgment and the flexible use of a wide range of clinical flow processes relating to access to care across multiple clinics and specialties. Some of the major duties of this position include, and are not limited to:

• Schedule appointments in a timely manner. All appointments will be made with the patient's input, either in person or by phone. This may require a high level of coordination to avoid patients and sending sites not being able to conduct their CVT appointments.
• Daily review of active/pending consults, Recall list, and communications for accuracy and disposition.
• Monitor pre-appointment requirements to assure readiness for virtual (CVT/VVC/phone) visits.
• Incumbent will use the appropriate scheduling platform for scheduling needs. Which includes CRPS, VISTA, VSE, CTM, and/or TMP, when applicable.
• Work with the interdisciplinary team to schedule follow-up visits for inpatients and outpatients in the CRH Clinics with various providers and in a variety of modalities (face-to-face, VVC, and phone if needed).
• Coordinate care of Veterans at other VA facilities within the VISN.
• Obtain appropriate inpatient and outpatient records and ensure they are appropriately entered into the VA computerized system to ensure availability for patient care.
• Ability to operate computerized programs and databases to enter, modify and retrieve sensitive information/data into or from electronic medical records, scheduling systems, and/or reports.
• Knowledge of basic medical terminology and more specific terminology as needed for problem­ solving and communication with Specialty teams.
• Ability to communicate effectively and professionally with Veterans and employees at varying grade levels.
• Ability to identify concerns, perform the tasks required to resolve the issue accurately and timely, and follow up as necessary to ensure a satisfactory resolution.
• Ability to make appointments in a clinical setting.
• Ability to work independently in the accomplishment of a wide variety of duties, including setting priorities and coordinating work within a complex range of Primary Care, Mental Health, and/or Specialty CRH clinics.

The incumbent is accountable to the Primary Care Social Work Supervisor in carrying out operational functions described above. The incumbent has clinical involvement in patient care by providing clinical case consultation to patient/family/community inquiries about services or patient care issues provided by the VA. Clinical social work services may be delivered in person or by electronic media, such as phone, V-Tel or home tele-health. Services will generally be provided in a clinic setting but may occasionally need to be provided in the community or in a patient/family home.

Psychosocial Assessment - The social worker will develop an assessment of the veteran, family, economic, community, and psychosocial support
systems when clinically indicated. The assessments must reflect a high level of skill in gathering, analyzing, interpreting, and documenting data
that is pertinent and directly related to patient health care planning, treatment, education and case management. The veteran, family members, significant others and the interdisciplinary treatment team are involved in this process, whenever possible. The goal of the communication is to highlight the veteran's strengths, limitations, and internal/external supports and service needs in order to optimize the veteran's functional status and safely maintain the veteran in his/her home.

Clinical diagnosing using DSM-5 and ICD-10 behavior codes - Incumbent demonstrates knowledge in DSM-5 diagnosing and documents diagnostic
impressions in psychosocial assessments when appropriate. Clinician may function independently or within a team to determine an appropriate
diagnosis and formulate a treatment plan based on that diagnosis.

Treatment Planning/Goal Setting - Incumbent demonstrates basic knowledge of psychosocial treatment modalities and demonstrates the ability to implement treatment modalities in working with individuals, families, and groups to achieve treatment goals. This requires judgment and skill in utilizing supportive, problem solving, or crisis intervention techniques. The social worker is responsible for developing a treatment plan, and setting achievable treatment goals with the veteran/family in collaboration with the primary care interdisciplinary treatment team members when indicated. Incumbent demonstrates basic knowledge of medical and mental diagnoses, disabilities, and treatment procedures to include acute, chronic and traumatic illnesses/injuries; common medications and their effects/side/effects; and basic medical terminology.

Therapy- Social worker will provide the veterans and their caregivers with ongoing psychotherapy (group and/or individual) and/or supportive counseling when appropriate. Incumbent is able to demonstrate the ability to utilize the DSM-V to ensure appropriate treatment recommendations.

Referral to service providers - Throughout the course of treatment the social worker is the subject matter expert on VA and/or community resources. The social worker will collaborate with other service providers in reassessing the veteran's needs for VA and community resources and entitlements. The social worker is responsible for educating the veteran and/or caregiver of the available services and assisting them in establishing the appropriate referrals based on the veteran's preference or that of his/her surrogate decision-maker.

Resource Development - The social worker is responsible for developing a resource file of VA and community social service programs and will refer the veteran to needed services.

Advocacy - Social worker understands the intimidation of bureaucracy and will act as an advocate when it serves the best interest of the veteran/family. When appropriate and feasible, social worker will educate and encourage the veteran/family to advocate for themselves, thus fostering a sense of independence and empowerment.

Crisis Intervention - Social worker is experienced in making rapid assessments and developing crisis management plans to maintain patient in the home, for admission to acute, short term and/or long term placements. Social worker has access to multiple resource directories in addition to previously developed resources to meet the demands of a crisis.

Work Schedule: Monday - Friday, 7:30 a.m. - 4:00 p.m. or 8:00 a.m. - 4:30 p.m.
