The Advanced Medical Support Assistant (AMSA) interact with both internal and external customers providing exceptional customer service and ensuring overall customer satisfaction. The Chief Nurse executes leadership that is characterized by substantial and continuous responsibility and accountability for population groups or integrated programs that cross service and/or discipline lines, influence organizational mission and health care. The Nurse IV is responsible for administrative functions of their assigned nursing units/programs. Functions as an administrator, educator, and consultant utilizing management theory in collaboration with VISN, Chief of Staff, Patient Care Services and other services. The Dialysis Vascular Access RN performs in a specialized area of nursing that focuses on managing a caseload of renal outpatient and inpatients through close coordination of care with multiple team members. The role requires comprehensive knowledge of the hospital system, and community resources, familiarity with personnel in multiple departments, the ability to relate well with patients and families in stressful situations, and the ability to advocate for the best interest of the patients with all clinical staff.

Assesses care coordinator identifies the vascular access needs of the patient with renal failure. Procedures, delivers, and coordinates services for the patient in the dialysis facility, outpatient clinic, and hospital setting. Provides for ongoing monitoring of the vascular access to ensure adequate dialysis is obtained, maximum life of the access is achieved and plan for the future intervention area coordinated when necessary. Collaborates with the Vascular Access surgeon and clinics in the coordination, management, and outcomes of patients requiring vascular access for dialysis. When assigned the Vascular Access Care Coordinator nurse collaborates with the Transplant Program and is responsible for screening potential donors and working closely with the medical team and transport services.

The Dialysis Vascular Access Care Coordinator nurse possesses a strong core knowledge base in nursing and has expanded clinical knowledge in the area of nursing expertise. RN staff focuses on providing and arranging for the education of patients approaching End Stage Renal Disease, preparedness and education for permanent vascular access, and preparedness for modalities of dialysis. The role requires extensive knowledge of all forms of renal replacement therapy and experience of dialysis therapy. The Dialysis Vascular Access care coordinator nurse is responsible for collaborating with services internal and external to the VA to facilitate care transition to effectively meet the patient's needs. RN staff provides professional assessment, coordination, and planning of health care services, and acts on behalf of the veteran to ensure the necessary clinical services are received and that progress is being made.

• Performs program analysis, development, review, research, design, and contact work as possible with various agencies (public, DOD, state, etc.).
• Develops and implements information systems to track service activities including visits, referrals, patient demographics, equipment utilization, housing, and patient satisfaction. Develops, implements, and evaluates clinical guidelines and protocols to establish appropriate utilization of services.
• Responsible for submitting reports as required by the Medical Center, the VISN, and Central Office. These reports include monthly and annual reports as required.
• Administratively responsible for coordinating services between assigned programs to reduce barriers to care and increase access across multiple care lines and locations.
• Provides program oversight and monitors programs to ensure programs meet The Joint Commission (T JC) and Commission on Accreditation of Rehabilitation Facilities (GARF) accreditation requirements; ensures programs align with agency mission and strategic goals; initiates and leads performance improvement activities; and ensures programs incorporates stakeholder feedback.
• Performs other duties as assigned.

The Supervisory MSA is responsible for managing and supervising the appointment management services within OCC and supervises one Lead Advanced Medical Support Assistant and ten Advanced Medical Support Assistants (MSAs) within the program. He/she collaborates with and has an extensive knowledge of all clinical applications, services and programs, to include but not limited services within the OCC. The Supervisory MSA is flexible, multi-skilled and creative in solving everyday challenges unique to this standalone service. The Supervisory MSA assigns work and daily priorities to the Lead and Advanced MSAs and ensures the goals and mission of the Office and Health Care System are being met. S/he is responsible for all performance management functions related to Lead and Advanced MSAs.

The Supervisory MSA applies principles, methods and guidelines to evaluate administrative operations in order to determine best practices for organizing administrative work and the flow of information throughout the service. S/he must take into consideration the staff ratios and skill mixes needed to accomplish the work. Analysis results in recommendations for improvement and implementation of new systems and processes that are discussed with the rest of the management team for decisions regarding the efficiency of service operations. The Supervisory MSA performs duties related to the receipt, intake and indexing of health and administrative information.

The Supervisory MSA will interface with personal computer (PC) applications as well as numerous Veterans Health Information and Technology Architecture (VistA), applications. The Supervisory MSA will serve as a backup for time and leave management processing. The Supervisory MSA must possess a high degree of tact and diplomacy when dealing with Veterans who have multiple medical problems and may be frustrated with the administrative process associated with coordinating treatment with community care providers.

The Supervisory MSA provides a variety of complex and unique duties in support of the MISSION Act. The Supervisory Advanced Medical Support Assistant will conduct and lead daily team huddles to assist with the coordination of patient care management, deploying customer contact and call center strategies, monitoring performance of the unit and make recommendations for improving the Veteran access to care and experience with VA and community care providers. The Supervisory MSA plans and directs programs within the Office of Community Care. The Supervisory MSA is responsible for implementing and deploying operational changes when necessary to ensure care in the community is provide in accordance with MISSION Act guidelines.

The Supervisory MSA has full supervisory responsibility of at least one lead MSA and ten Advanced MSA's. Assignments at this level include but are not limited to; assigning and evaluating the work of subordinate staff, resolving complex problems to ensure patient requirements are met. Analyzing staffing and workload patterns to ensure appropriate coverage of workload needs. Planning work to be accomplished by subordinates, setting and adjusting priorities. Determining appropriate tours of duty. Approving or denying leave. Evaluating performance, rating employees, and recommending performance awards. Interviews and selects subordinate staff. Advises Program Manager with process improvement suggestions and new strategies by extracting and analyzing data to better manage care access and ensure compliant appointment management practices per the MISSION Act and scheduling directive guidelines.

The Supervisory MSA manages CPRS view alerts from providers, correspondence from external community care affiliates, caregivers, family support staff and Veterans for needed actions involving patient care and follow-up. Ensures subordinate Lead and Advanced MSAs appropriately manage inquiries and requests for care in the community to ensure requests are routed to the appropriate external provider and scheduled timely in accordance with MISSION Act and VA guidelines. Supervisory MSA has full administrative and professional responsibility for planning and directing the Lead and Advanced MSAs activities. In certain situations, the Supervisory MSA will be required to conduct scheduling assignments if no Lead MSAs or AMSAs are available to schedule patients or if the situation is complex or emotionally or politically charged.

Major Duties include:

The Primary Care Provider (PCP) Float will provide primary care services in support of primary care operations in the San Antonio service area. As a float provider, this position will provide support to Patient Aligned Care Team (PACT) at an assigned San Antonio Clinic and may be detailed to other VA Clinics in San Antonio as staffing and workload dictate.

THE PCP Float may be asked to provide coverage for a vacant/absent PACT provider to supplement PACT services by delivering same-day care, routine scheduled care, or new patient intake for PACT panel(s).

The Primary Care Physician is responsible for providing Comprehensive Primary Care for Veterans and should, in the context of a longitudinal relationship, fulfill all primary care needs, including acute and chronic illness, gender-specific PC, mental health (Screen for conditions such as depression, substance use/abuse, military sexual trauma and PTSD, and evaluate and treat uncomplicated mental health disorders as well as substance use disorders and refer patients appropriately for further evaluation and treatment of sexual assault and military sexual trauma, mental health disorders and substance use disorders) and preventative care (age-appropriate cancer screening e.g., colon, breast and cervical cancer - as well as weight management counseling, Chlamydia screening (when indicated), smoking cessation and immunizations, etc.) along with coordination of care delivery (when specialty care is needed, the primary care provider will coordinate this care and communicate with the specialty provider regarding the evaluation and treatment plan to ensure continuity of care).

The PACT Model is a patient-driven, team-based approach that delivers efficient, comprehensive, and continuous care through active communication and coordination of health services through an interdisciplinary team that includes the PCP, RN Care Manager, LVN, clerical associate as well as other clinical services necessary to meet the health goals and needs of the Veteran patient including specialists. While the Primary Care provider's relationship with the Veteran patient will remain key, that bond will be augmented and strengthen by involving other health care staff as part of an integrated team. Together the team takes responsibility for the Veteran patient's ongoing care through a cooperative effort among all members.

Alternative or collateral duties may include management of a subsection of primary care patients, the subsection may be defined either by a disease status, or demographic status (e.g., women veterans, OIF/OEF veterans, employees, etc.). Typically these duties are designated at the time of joining but occasionally during the course of employment, participation in special projects pertinent to assigned section may be necessary as the projects become available through strategic or other initiatives.

Physicians may be assigned duty as a supervising or collaborating physician for one or more mid-level providers. Physicians are expected to provide clinical oversight and review the care provided by mid-level practitioners. Most providers will need to act as surrogates for the patients of their colleagues, when one or more of their colleagues are on leave.
