1. Performs nutritional assessment in newly admitted HBPC patients to identify patients at nutritional risk, to obtain pertinent information on eating patterns, economic situation, cultural background, compliance/comprehension of previous diets and other factors influencing nutrient intake and food tolerances/preferences and preparation. Provides initial and on-going nutritional assessments of receiving central nutrition. 5. Orders labs, diets and consults per Scope of Practice/Clinical privileges.

Community Care RN CoordinatorNurse II handles the process of organizing and coordinating resources and services in response to individual healthcare needs along the illness and care continuum and in multiple settings. The goals are to center services around the patient, to foster patient self-managed care, and maximize efficient and cost-effective use of health resources. SCI/D Home Care Nurse Functions:

• Demonstrates a high level of clinical expertise, leadership, and creative approaches to management of the complex care of patients with Spinal Cord Injuries/Disorders.
• Responsible for program development and administration of the SCI/D Home Care program, in accordance with VA Directives, Handbooks and all applicable regulatory and accreditation standards.

Major Duties:

• Plans, coordinates, implements, monitors and evaluates the delivery of services within the scope of practice and administrative responsibility of the assigned program.
• Monitors the efficiency, effectiveness, timeliness and quality of services offered within their scope of practice and area of responsibility pertinent to assignment.
• Develops program guidelines and initiates new programs. Provides data and progress reports about the assigned area. including quality management performance improvement monitors and any statistical or data analysis reports requested by management pertinent to assignment.
• Incumbent must be completely conversant with all levels of social work treatment and skill; and with group theory and practice, including new developments in the field.
• Participates in the development of department policies and procedures to further social work goals and objectives; and in setting up the structure to ensure their implementation.
• Incumbent must have a comprehensive knowledge of the guidance/mentoring process and method; including understanding of individual learning theory, learning in groups and systems theories.
• Formulates individual development plan consistent with performance appraisal form and Service guidelines with all staff assigned to him/her.

The Staff General Dentist serves as a practicing dental provider that performs a full range of professional dental procedures in connection with the treatment of commonly encountered dental diseases or dental health problems requiring standard corrective, restorative, or preventive measures.

The staff general dentist shall be proficient in all aspects of general dentistry including the ability to perform selected specialty services. He/she is fully mentally and physically capable of performing these essential functions of a general dentist without impairment. Specifically, the provider should possess the procedural expertise and competency to perform the following:

1. Make oral and radiographic examinations of the oral cavity.
2. Diagnose pathological or irregular era( conditions.
3. Administer local anesthesia,
4. Perform routine and complex restoration of teeth.
5. Perform simple and complex extractions of teeth,
6. Perform alveoloplasty and other routine surgical procedures.
7. Perform prosthetic dentistry to include immediate dentures, full dentures, partial dentures, crowns and fixed bridge work.
8. Perform routine endodontic procedures on single and multi-canal teeth.
9. Perform routine and surgical periodontal procedures.

The major duties and responsibilities include, but are not limited to:

• Lifecycle management of all medical systems such as imaging, life support, dialysis, and monitoring to include physical and network support.
• Coordinates and works effectively with other facility support departments including but not limited to Facilities Management Service (FMS), Office of Information Technology (OIT), Patient Safety, Sterile Processing Service (SPS), Logistics, Contracting, Informatics, and Environmental Management Service (EMS). Specific tasks include but are not limited to incoming inspections, regular and unscheduled maintenance, and planning for end of life replacement in conjunction with clinical services. Utilizes common test equipment including, but not limited to patient simulators, pulse generators, network analyzers, test phantoms, electrical safety analyzers, oscilloscopes, and multimeters.
• Maintains functional knowledge of relevant safety codes and standards of regulatory bodies including, but not limited to, The Joint Commission (TJC), National Fire Protection Association (NFPA), Food and Drug Administration (FDA), Nuclear Regulatory Commission (NRC), Association for the Advancement of Medical Instrumentation (AAMI).
• Documents all work performed in the computerized maintenance management system (CMMS). This documentation is completed in accordance with regulatory body requirements, the Office of Healthcare Technology Management (HTM) Documentation of Services policies and guidelines, and other VA requirements.
• Applies basic networking principles to resolve medical system communication issues including, but not limited to, node to server failures. Adheres to VA Medical Device Cyber Security policies and procedures in pre-procurement, during system implementation, and throughout the life of the system.
• As assigned, functions as a Contracting Officer Representative (COR) and keeps appropriate certification levels.
• Investigates, remediates and documents Repairable Medical Device (RMD) recalls and safety alerts issued through the VA Alerts and Recall Management System (ARMS) within established timeframes. Participates in local safety improvement processes such as Root Cause Analysis (RCA), safety work groups, and Environment of Care Rounds. Identifies hazards during routine job performance and takes action to ensure they are addressed and corrected.
• Provides training and education as appropriate to medical technology end users and clinical staff.
• May be tasked with leading and managing installation or upgrade projects for medical devices and systems. This includes but is not limited to pre-implementation planning and coordination with end users, vendors, and other onsite support staff (OIT, Informatics, SPS, etc.)
• Additional duties are performed as assigned by supervisor related to maintenance, remediation, and tracking of medical systems (action items, special projects, etc.) to meet mission critical requirements. May be asked to assist in other administrative duties as well, including participating in hiring panels.
