An exciting and rare opportunity has arisen for aBand 7 Clinical Nurse Specialistto provide support and clinical leadership to Nurses in the Community Children's Nursing Team in Harrow.
The post holder will be expected to be an experienced and motivated children's nurse and will work closely with the other Clinical Nurse Specialists within the service. At the same time, support senior management in the delivery of care and support, and deputise when required.
This role also requires the post holder be mobile, although the main base for this post is atCaryl Thomas Clinic.
The post holder will be supporting a team of Qualified Nurses delivering support/intervention to children and young people with acute and complex medical and health needs. The post holder will be responsible for developing and standardising training resources, standard operation procedures, provide clinical oversight and supervision to staff.
The post holder will have close working relationships with the other Clinical Nurse Specialist and also establish professional working relationships with Harrow's Allied Health Professionals, Community Paediatric Consultants and peers across within the Trust and within Universal Services. The service links closely with the local authority social care team (Children with Disabilities Team) and SENAS team.

• Work as part of the Community Children’s Nursing Team, in conjunction with the Community Children's Nursing Clinical Manager to manage the resources of the team, including staff, in line with professional standards and quality of care. This includes provision of clinical leadership to a team of Community Children’s Nurses (CCN), Special School Nurses (SSN) and students.
• As a Band 7 CCN Clinical Nurse Specialist , you are expected to take responsibility for the assessment, management and evaluation of care to children with acute and complex health needs. The post holder will be a highly visible presence in clinical practice, both within the CCN and SSN team and responsive. Contribute and maintain high standards of care and practice to improve children’s and young people’s care across the Community Children’s Nursing Team and the quality of the services we provide by:
• keeping the people who use our services as safe as possible through the use of sound clinical skills and effective risk assessments.ensuring the best possible clinical outcomes.
• using up-to-date skills and adhering to evidence based policies and procedures
• ensuring the people using our services have a good experience.
• Respecting, empowering and working in partnership with people throughout the care planning process
• The post holder is also required to provide managerial cover in the absence of the Clinical Nurse Manager.
KEY DUTIES/RESPONSIBILITIES
Clinical Duties:
• Take continued responsibility for the assessment, management and evaluation of skilled nursing care in negotiation with the child and family
• Work in a collaborative manner with other agencies to ensure a seamless delivery of care to families.
• Provide information and support to facilitate the child’s and family’s own choices with regards to nursing care
• Promote Health Education as an integrated aspect of care delivery.
• Lead complex discharge planning and assessment of a child’s needs, ensuring detailed communication with discharging hospital and safe discharge for the child and family.
• Responsible for ensuring that staff perform effectively and safely meeting the requirements of the post, taking appropriate action as necessary
• Maintain accurate, systematic and timely record keeping including recording of clinical data.
• Provide skilled, effective, evidenced based nursing care for children with complex care needs including technology dependent such as tracheostomy care and central venous access.
• Participate in an on-call rota with the Clinical Manager when a child is receiving end of life care at home if needed.
• Manage the safe administration of cytotoxic drugs in the community and ensure safe handling of body fluids and contaminated sharps in personal practice and within the team.
• To work as an autonomous and lone specialist practitioner.
Management and Leadership:
• Provide supportive and facilitative leadership to all Band 6 Community Children’s and Special School nurses.
• Provide managerial cover in the absence of the Clinical Manager.
• Support the Clinical Manager in managing the team’s resources, including staff, in line with professional standards and quality of care.
• Lead projects as agreed with the Clinical Services Manager, in line with CNWL and national strategies, and personal skills and interest.
• Be aware of the financial resources of the Team and budgetary constraints.
• Identify and lead on Safeguarding Children procedures including attendance at Case Conferences.
• Establish and maintain good relationships with the relevant professionals within Community Services; Acute services, Special schools, Social Services and other agencies.
• Actively participate in the recruitment of staff and demonstrate good practice in relation to HR policies.
• Actively participate in the orientation and development of new staff to the community.
• Be responsible for the day-to-day management of the team ensuring adequate staffing levels are maintained and a high standard of care provision is achieved which meets client expectations.
• Lead in planning shift rotas.
• Lead projects as agreed with the Clinical Manager, in line with CNWL and national strategies, and personal skills and interest.
• Participate in Clinical Supervision of junior staff in line with CNWL Policy.
Professionalism:
• Maintain accurate records and statistical returns as required within the CNWL (Central North West London) NHS Trust, including entering data on the relevant patient electronic recording system.
• Actively participate in team, organisation and other relevant meetings.
• Be aware and act in accordance with the Nursing and Midwifery Council Code of Professional Conduct.
• Attend regular clinical and safeguarding supervision in line with CNWL Policy.
Teaching and Training:
• Empower parents and carers to enable them to safely care for their child within the community environment through education, support and risk assessment.
• Participate in reflective self-appraisal and Personal Development Planning.
• Maintain up-to-date knowledge and skills and undertake education in accordance with individual, team and CNWL NHS Trust needs, within a framework of a Personal Development Review.
• Facilitate reflective self-appraisal for designated team members.
• Promote an active learning environment and be actively involved in meeting the educational needs of learners.
Clinical Governance:
• Take appropriate action in regard to any accident or incident to a child, family member, staff member or visitor.
• Be responsible for the care and safe-keeping of equipment issued, ensure servicing and reporting of any defect or loss.
• Ensure that the service is delivered in line with the specifications within the relevant Service Level Agreement, ensuring excellent lines of communication are maintained at all levels of the Trust.
• Ensure personal and team safety by adhering to the CNWL policy on lone working.
