The post holder will be based within the Urgent Care Bureau Team, and working with the Urgent Community Response Team clinically, when required. The post holder will have operational and clinical responsibility of the Urgent Care Bureau Team.



The post holder will be able to provide clinical expertise in Triage, In-reach and Virtual Wards across the Adult Community Services Division.



The purpose of the role is to work across services and with wider system



The Post holder will be expected to participate in working with secondary care, to identify and assess patients suitable for community services, and to prevent avoidable hospital admissions and facilitate early supportive discharge. The work will be undertaken collaboratively with Secondary and Primary Care, and Community Services, including Urgent Care Response, Early Intervention Community Teams (EICT), Urgent Care Bureau and other services deemed in scope.



The post holder will also attend multidisciplinary team meetings (MDT’s) to provide advice and support to the Virtual Ward (Hospital @Home) Model.

You will be required to support staff in the service

Experience of care in a community setting would be an advantage but not essential. You must be able to travel around the city during the working day.

The main purpose of the Multi-Skilled Assistant is to deliver high quality, caring and compassionate patient care. The post holder will work across the disciplines of nursing, physiotherapy and occupational therapy, carrying out assigned tasks involving direct care as part of the Urgent Community Response team. Key Responsibilities for Registration Authority support element of the role:
1. Liaise with other Departments as required, e.g. DTS, Electronic Staff Records (ESR)

· Responsible for receipt of documents electronically, inputting onto TRAC, Staff Record (ESR), NHS Jobs and relevant external publications via advertising agency. Set up recruitment file. Ensure that all internal recruitment processes are followed accurately and in a timely manner, highlighting any exceptions or problems to the Senior Recruitment Officer or Recruitment Manager and ensuring the International Recruitment team are kept updated.

The clinical role is based within Urgent Care/SPA supporting the clinical triage of new patient referrals and messages under the supervision of the Clinical Team Lead. The post-holder will be accountable for the delivery of high quality care to patients within their own homes and or other community settings, preventing avoidable hospital admissions. Working with other members of the wider team to provide high quality care and to promote health/well being and independence for patients on the team caseload. As part of this role rotation to the Early Intervention Care Teams across the city will take place every 4-6 months.



The post holder will not have any direct management responsibility; they will however provide education and supervision to student nurses or registered nurses involving training in regards to triaging and dealing with messages via phone.

Applications are invited from dentists on the specialist list in Special Care Dentistry to join our clinical team providing services across the Birmingham and Black Country area.

As well as providing specialist care for adults with disability, medical or mental health problems, this post will involve appropriate lead roles delegated by the Head of Service to enable development of a portfolio of post specialist training leadership and management competencies.

You will need substantial experience at specialist level in Special Care Dentistry including provision of IV sedation and treatment under general anaesthesia; also experience in oral surgery attained through a hospital based oral surgery post of at least 6 months. You will need excellent presentation skills, and experience in service leadership and project management skills.

You must have full GDC registration and be on the Specialist list in SCD (or within 6 months of expected CCST); you must also have a registrable postgraduate qualification in SCD eg. DSCD, MSCD, MSc and have entered or be eligible to be entered onto the local NHS performers list.

You will also need to be flexible and able to work at different sites across the Birmingham and Black Country area.

• Assist in the maintenance of the Certero Software Asset Management (SAM) system,
• Ensuring the asset inventory is managed accurately, whereby all new devices are added to the system and decommissioned devices are correctly archived from the system and the associated software licenses released.
• Monitoring and managing IT assets and inventory, utilising the SAM application (including asset allocation) for the distribution and disposal of their lifecycle.
