Providing specialist expert advice to the SIRO, Caldicott Guardian to ensure that the Trust operates within legal and ethical frameworks. Working alongside a Mental Health Liaison Practitioner, you will work across the interface between Primary Care and Mental Health Services to facilitate mental health assessment in primary care and support GPs with joint decision making in relation to secondary care referrals, signposting, and transition of service users from secondary mental health services back to Primary Care. Your expertise will help inform GPs management plans for those with mental health needs in primary care.

Providing support, education and problem-solving approaches for adults of working age, you will be an active member of the primary care team with the ability to undertake very short-term interventions for clients with mild to moderate mental health difficulties.

This is not a ‘case load’ role or service, enabling you to be flexible and dynamic in a well-supported autonomous role that will allow experience, decision-making skills and service user focus to thrive and develop.

We are excited to advertise a Healthcare Assistant post for our Acute Assessment Ward, OAK 3, based at the Edith Cavell site. The Cavell is an accessible site with free parking, a café on site and a beautiful modern design with new wards.

At CPFT our adult inpatient wards operate an exciting, ambitious model of care, known as the 3-3-3 model, focussed on providing the right care at the right time. The three wards work together to provide the best care and support possible; Oak 3 our Acute Assessment Unit, where patients stay up to 3 days, Oak 1, our Treatment ward, where patients stay up to 3 weeks, and Oak 4, our Recovery Unit, where patients stay up to 3 months.

We need a charismatic, energetic, enthusiastic health care assistant to support the provision of evidence-based treatment and work with our multi-disciplinary team to support our service users in their recovery.

You may be required to undertake and successfully complete the new national Care Certificate within a 12 weeks’ timeframe if you meet our criteria. You will receive full training and support and The Care Certificate sets out 15 national care standards that must be achieved by all health and social care staff to demonstrate that they have the skills and knowledge needed to provide safe and excellent quality of care. Social Care Direct is the first point of contact for all new referrals to Wakefield Council. Your role would be to triage these referrals, providing urgent responses, offering advice and information and signposting. The role involves triaging requests for social care assessments and safeguarding referrals. As a Level 2 Social Worker you would also have the chance to attend multi-agency meetings and be a lead in championing new initiatives and team developments. Most of our work is done over the phone, so you will need excellent phone skills.
