Act as a role model to colleagues, seeking the highest standards of professionalism. Use their initiative and take responsibility for themselves and the quality of their work and the service they provide to patients. Assess the care needs of individual patients using evidence based/patient centred principles to plan, implement and evaluate interventions Facilitate 1 to 1 and group interventions. Allocated named patients, ensuring timely review of care plans and assessments. Provide leadership to the team, supervising, monitoring, supporting and educating them in good professional and clinical practices. Assist in maintaining high standards of patient care, ensuring patients individuality and privacy are maintained at all times. Demonstrate effective handover and communication of care Maintain an up-to-date knowledge of all Trust policies and procedures. Adhere to the NMC/OT/HCPC code of Professional Conduct.
