Work flexibly across all Intermediate Care Services sites to meet the needs of all discharges and transfers of care for all adult patients being discharged or requiring community support. Drive communication and response to maintenance of patient flow in support of organisational targets. To ensure you are an integral part of a responsive team are delivering a highly responsive and effective service delivery. Complete a daily work allocation and ensure completion of all tasks to effectively operate the service, within this demonstrate a flexible approach to meet individual patients/family need. Report & monitor problems with patient transport services and escalate issues to senior clinical flow team. Provide proactive contribution to allocated wards to ensure length of hospital stay is determined by clinical need only and not by organisational resources. To promote and maintain effective communication channels between patients, their relatives, and relevant teams in order to successfully coordinate discharges. Proactively advise and support staff to initiate the early discharge planning of patients with simple needs ensuring appropriate assessments have been completed in a timely manner to facilitate discharge. Identify, progress chase and monitor delayed discharges and lost bed days within the patient journey and referrals. Screen and input information of onward referrals made to the homes and health and social teams, including the completion of trusted assessment documents. Maintain accurate records and participate in audits as required in relation to the discharge planning process. Support and respond to the Trust Escalation Processes, as guided by the IDS Business Manager, Head of Site Operations & Flow / Head CCM/ Senior Directors / Managers. Ensure compliance with other related Trust Policies and Department of Health Legislation regarding discharge planning processes.

We are searching for a prospective Anti-Social Behaviour Officer to join a team based in Sittingbourne on a full time, temporary basis with an initial contract of 3 months. The ASB Officer will provide a high quality, proactive, responsive, and prompt ASB management service to residents to effectively tackle all forms of hate crime, neighbour nuisance and anti-social behaviour using a broad range of interventions, including the legal framework. This role offers remote working with 1-2 days a week in office or conducting visits where necessary.
Requirements
The ideal candidate will have the following skills and experience:

• Previous experience in a role involving ASB Management, with high level cases of ASB
• Knowledge of housing, civil, and criminal law, including the Anti-social Behaviour, Crime, and Policing Act 2014
• Ability to take responsibility for own safety and create a safe working environment
• Empathetic and respectful towards residents/customers, value diversity
• Able to understand and gather relevant information to process legal action and ensure a successful conclusion.
• Experience of presenting cases at Court to support the ASB process.
• Experience of achieving positive results in dealing with ASB and tenancy services.
• Experience working with CRM and/or React are desirable

Role Expectations

• Effectively manage and deal with a delegated case load of anti-social behaviour, hate crime and domestic violence cases
• The role involves investigating complaints, maintaining appropriate records, and overall providing excellent customer service in all areas
• Provide proactive support to the Neighbourhood Housing team to identify and investigate instances of anti-social behaviour, promptly assessing whether preventative intervention or legal action is required
• Actively participate with external agencies to identify where preventative intervention is required, ensuring a prompt and joined up approach in tackling anti-social behaviour and tenancy enforcement
• Where legal action is required, progress cases in accordance with legislation. This includes making decisions on the appropriate level of action to be taken, such as possession proceedings and injunctions, gathering evidence, preparing cases for Court, obtaining witness statements, attending Court and supporting witnesses
• Take and draft witness statements in preparation for legal action, arranging support and protection (if required) for witnesses

Job Purpose

• To undertake surveys and joint visits with Occupational health staff of dwellings in response to applications for Disabled Facilities and other forms of housing grant assistance and determine most appropriate and cost-effective options.
• To produce detailed accurately measured schedules of works and small-scale design and drawing of sufficient quality that the applicant's chosen contractors are able to accurately and competitively estimate the cost of the grant-aided works.
• Using the Council's approved Schedule of Rates and builder's estimates supplied by the applicant, to accurately determine the eligible expense of the grant aided works.
• To carry out site inspections whilst works are in progress, and following completion of works to ensure that all works completed are of the correct quantity and are of the quality demanded by the Council.
• Calculate payments of grant and prepare payments and completion certificates for authorisation.
• Quantify works carried out, and any additional, unforeseen works that may be necessary to meet with the objectives of the grant and to recommend approval, where appropriate, of variations up to a limit of the maximum grant and to advise the Manager/ Officers of instances where this limit is likely to be exceeded.
• To provide, throughout the grant process, appropriate and accurate information and advice to applicants and their agents regarding their rights and responsibilities under grant legislation and associated Council policies and procedures. Liaise with the applicants chosen architect in order to ensure that the objectives and limits of grant aid are considered when plans are drawn.
• To liaise with officers from other disciplines, e.g. Occupational Therapists, Planning and Building Control Officers and to have regard to their requirements in designing the schedule of works.

Qualifications

• City and Guilds - NVQ Level 3
• Current and comprehensive knowledge of building construction and ability to identify defects and write schedules of work.
• Ability to evaluate and solve problems.
• Computer literate with the ability to use various standard and nonstandard programmes.
• The ability to communicate clearly and effectively is essential to the role, adapting communication style to suit differing audiences and provide advice in accurate spoken English is essential.
• The normal duties will involve travel on a regular basis a full and clean driving licence and have car available for use for work is essential.
• Knowledge of disabled facilities grants and appropriate laws relating to grants and construction.
• An understanding of the - Housing health and safety rating system (HHSRS), Decent homes standards.
• Experience of working with legislation and ensuring that work meets the requirements.
