The Advanced Practice Nurse (APN) III Certified Nurse Practitioner (NP) practices in Surgery Service to provide a full range of services emphasizing quality, timeliness, and the advancement of the Veterans Affairs (VA) mission. The HBPC RN is responsible and accountable for individual nursing practice and determines the appropriate delegation of tasks consistent with the nurse's obligation to provide optimal outpatient care. This HBPC RN provides direct Veteran care and care coordination, as it pertains, to Veterans and their care within the HBPC Program. Responsible for case management, assessment, triage, Veteran education, resource coordination, medication and treatment coordination, and care planning.

This HBPC RN plans and delivers technically competent care in the home of Veterans with complex needs, including health promotion, health prevention, geriatric care, primary care, and palliative care. Utilizes interventions, which maximize health capabilities and reduces unnecessary urgent care and emergency room visits, per Veteran's goals of care. Makes home visits to Veterans in the program. The focus is on individual growth and development in HBPC nursing practice with increased levels of self-direction. Seeks guidance on how to improve care provided to patients and families. Delegates elements of care to the appropriate individual based upon assessed needs, condition, and safety of the patient. The HBPC RN brings HBPC principles and theory with limited application of critical thinking skills.

This position potentially requires flexibility in schedule and assignments as this is a 24/7 hospital. HBPC RNs must have a current, valid, unrestricted driver's license and the ability to operate a GSA vehicle for community home visits and outreach activities.

The Primary Care Physician Assistant in collaboration with the interdisciplinary team, manages the care of outpatients in the Ambulatory Care Service as a Primary Care Provider as part of a Patient Aligned Care Team. Primary Care is provided in Rockford, Freeport, and Janesville Community Based Outpatient Clinics.

Scope of Practice, duties may include but are not limited to:

• Performs comprehensive history and physical examinations and initiates treatment when indicated for acute and chronic medical conditions.
• Orders diagnostic laboratory and imaging tests as needed. Interprets and applies findings to make a medical diagnosis.
• Uses prescriptive authority granted by licensure to prescribe and administer routine therapeutics including medications, devices and procedures.
• Uses electronic record for documentation. Initiates consultation requests and records required notes on all procedures or patient visits in the in the electronic medical record. Participates in peer review and process improvement
• Performs procedures that are delineated by scope of practice. Emergency procedures may be required in situations where the physician is not immediately available and may include initiation and assisting in management of cardiac arrest, acute respiratory failure and first aid for life threatening injuries.
• Provides customer focused and cost-effective approach to all aspects of patient care.
• Educates and counsels veterans and families for promotion and maintenance of health as well as prevention of illness and disability. Instructs patients and families concerning treatment, medications and reportable signs and symptoms associated with their diagnoses.
• Ensures that the practice is consistent with Joint Commission, CARF, VA policies and procedures, clinical practice emergency medicine guidelines, VISN standards, national statutes and regulations, medical record documentation, mandatory continuing education, and HIPPA policies.
• Engages in systematic evaluation and improvement of the quality and effectiveness of patient care.
• Expected to act as a surrogate for the patients of colleagues, when one or more colleague is on leave.
• Timely documentation is expected and necessary in all areas in accordance with facility policy. All administrative work should be completed in a timely fashion.

• Provide comprehensive assessment, diagnostic work-up and clinic intervention, including comprehensive patient assessment, health history, physical examination, and patient education to Veterans in the Primary Care team setting; outpatients include both scheduled and unscheduled Veterans. Function as an integral part of a Primary Care multi-disciplinary team to coordinate, facilitate, and provide patient care and services.
• Discriminate between normal and abnormal findings to establish a diagnosis and initiate appropriate response to emergency health problems.
• Initiate appropriate referrals and consultations to other clinical services.
• Order and evaluate diagnostic tests as appropriate, recommend treatment and/or referral.
• Validate the effectiveness of previously implemented medical treatment.
• Initiate, evaluate, and change orders for medications.
• Include patient and family in planning care and establishing realistic measurable goals for health maintenance/improvement.
• Record/document health appraisal information, clinical assessments, treatment decisions, plan of care, ongoing management and therapeutic outcomes, order medications, diagnostic tests and referral consultations where needed for every patient encounter; recording will be accomplished using the electronic comprehensive patient record system (CPRS), and will be completed on the day of the encounter.
• Respond to computerized "alerts" indicating diagnostic test abnormalities and to health line requests from Veterans/family members on the day of receipt (weekends excepted).
• Follow Performance Measure guidelines, including completion of clinical reminders in CPRS. Access and timeliness standards; prescription order entry, National VA formulary requirements, and customer service standards.
• Work effectively with patients, families, members of the multi-disciplinary team, and other staff.
• Demonstrates flexibility in responding to clinic demands, adhere to personal and professional obligations and responsibilities, practice and maintain ethical standards, and sustain a work environment that is harmonious and customer service oriented.
