Delivering sessions across Lancashire and South Cumbria, you will work in the heart of the community. Session delivery will be face-to-face across the region.

What is the role about:

Reed Wellbeing Trainers will provide you with comprehensive, market leading training into the theory and application of our behaviour change based Path to Remission programme and other interventions, along with a thorough overview of service delivery and associated process requirements.

You will support participants through up to 12 weeks of total diet replacement, providing tailored interventions such as resilience, emotional eating and managing triggers. Following the total diet replacement, you will support participants through food re-introduction and weight management, focusing on nutritional advice, coping strategies and physical activity (where appropriate) to encourage a sustainable lifestyle. You will also support the monitoring of progress through regular blood pressure, blood glucose and weight measurements.

• Delivering Reed Wellbeing's individual assessments and 1:1 sessions in a professional, engaging and motivational style, in accordance with our curriculum, resources and quality

Merseyside Liaison and Diversion RECONNECT - Mental Health Practitioner

We are looking for a Mental Health Practitioner to join our Reconnect team within the Liaison and Diversion Team.

Liaison and Diversion is a process of supporting people coming through the criminal justice system, and diverting people from this pathway where possible through engagement with service users with vulnerabilities. A significant part of the role is liaising with criminal justice system stakeholders and colleagues to provide advice, guidance and support.

RECONNECT is a care after custody service that seeks to improve the continuity of care of people leaving prison or an immigration removal centre (IRC) with an identified health need. This involves working with them before they leave to support their transition to community-based services.

This post is subject to Police Vetting and an enhanced with children’s and adult’s barred list Disclosure and Barring Service (DBS) Check and full NPPV Level 2 Police Vetting.

Liaison and diversion and RECONNECT is a process whereby people of all ages with mental health problems, learning disabilities, substance misuse problems and other vulnerabilities are identified and assessed as early as possible as they pass through the youth and adult criminal justice systems.

You will be expected to work in court, police custody suites and HMP Walton and Altcourse. this role includes working across as well as the wider mental health systems and urgent care division.

You will be a registered practitioner within mental health (Allied Health Professions: Mental Health Nurse; Social Worker; Occupational Therapist; Learning disability Nurse) with demonstrable extensive post registration experience. You will have experience in undertaking specialist mental health assessments and have a sound knowledge of the Mental Health Act/Capacity Act and criminal justice system.

The post holder will work as part of the Occupational therapy team which are an integral part of a large health and wellbeing team at HMP Wormwood Scrubs. The team includes psychology and occupational therapy staff, speech and language therapist and substance misuse workers. The team contribute to the overall healthcare provision within the prison, working across the healthcare department to provide comprehensive OT input across the Inpatient ward, the Wellbeing Centre, neurodiversity unit and across the wider prison



The post holder will work within the mental health hub, a day service providing an Occupational therapy service to those with complex mental health problems, personality disorder, mood disorders, PTSD, trauma, and ADHD, as well as those on the autistic spectrum. The role will also have input to other elements of the healthcare services such as the inpatient unit, the in reach team and the health and wellbeing team.
1. To carry out standardised assessments, interventions and provide detailed feedback to the therapies and multidisciplinary teams about clients’ abilities, and also information about future plans and requirements for progress within a mixture of settings including but not limited to; the Wellbeing Centre, Neurodiversity unit, inpatient ward
2. To actively contribute to the multidisciplinary team’s risk assessment process and obtain agreement from the team before engaging service users in any activity that may require therapeutic risk taking.
3. To work in collaboration with other members of the multidisciplinary team to provide a coordinated care package.
4. To regularly review each service user’s progress in order to evaluate the effectiveness of treatment interventions and modify treatment plans accordingly.
5. To ensure that the OT package of care is being followed for all service users that postholder is responsible for including those of whom they supervise.
6. To work in accordance with professional guidelines issued by the College of Occupational Therapy which are relevant to the field of Forensic Psychiatry.

The post holder will receive regular individual supervision to support their practice, as well as opportunities to engage in reflective practice groups with the wider healthcare team at HMP Wormwood Scrubs. The team has a strong emphasis on support and training with regular in-house training opportunities as well as from external providers. Recent external trainings received by the team have been in hearing voices, working with trauma, mindfulness teacher training and personality disorder.

We are looking for a Mental Health Practitioner to join our Reconnect team within the Liaison and Diversion Team.

Liaison and Diversion is a process of supporting people coming through the criminal justice system, and diverting people from this pathway where possible through engagement with service users with vulnerabilities. A significant part of the role is liaising with criminal justice system stakeholders and colleagues to provide advice, guidance and support.

RECONNECT is a care after custody service that seeks to improve the continuity of care of people leaving prison or an immigration removal centre (IRC) with an identified health need. This involves working with them before they leave to support their transition to community-based services.

We are seeking an enthusiastic and motivated Advanced Therapy Support Worker to join us caring for patients in the community.

We are a responsive, dynamic, multi-disciplinary community team workingwith patients within their own homes, care homes, community support beds and in-patient setting, our service aims to prevent hospital admissions and facilitate discharges; as well as rehabilitation for patients.

Our integrated team comprises of Occupational Therapists, Physiotherapists, Advanced Therapy Support Workers and Generic Support Workers; as well as working closely with our Nursing teams and Social Services to provide an excellent service to our patients. This role is expected to provide cross cover support for our rapid response therapy team in the community and our rehabilitation beds locally in Oakland Village.


The successful applicant will have generic therapy assessment and treatment skills, be able to work as part of a team and will have support from a registered therapist.

Excellent communication skills are essential.

There may be a requirement to work within triage, to support the flow of referrals into the team and be able to liaise with patients/carers/MDT members and referrers from a wide range of services.

Access to and ability to use transport as travel between and across care settings will be required.

The community therapy service is provided 8am-6.30 pm 7 days a week.





This will include initial assessment and reviews of patients, creating specific patient led care plans and meet therapy goals in line with competencies and liaison with other specialist services within the Trust.

The post holder will be able to work independently and adhere to Trust Lone Working policies.

To communicate information to patients and carers regarding their health needs and associated treatment plans.

To report and take appropriate action regarding any change in patient condition or circumstance and to communicate verbal and written information to all relevant integrated care team members.
